Brakiyal Pleksus Yaralanmasında Robot Destekli Üst Ekstremite Rehabilitasyonu: Olgu Sunumu

Erb-Duchenne Palsi üst brakiyal pleksustaki lezyon nedeniyle ortaya çıkan kas zayıflığı ve motor fonksiyon kaybı ile karakterize bir hastalıktır. Rehabilitasyon kliniklerinde çeşitli konservatif rehabilitasyon yaklaşımları yaygın olarak kullanılmakla birlikte son yıllarda kullanılan robot destekli tedavi nörolojik bozukluğu olan hastalarda üst ekstremite fonksiyonunu iyileştirmek için umut verici bir yaklaşım olarak kullanılmaktadır. Bu çalışma Erb-Duchenne Palsi’li pediatrik hastada robot destekli tedavinin üst ektremite motor fonksiyonuna etkisini göstermek amacıyla yapıldı. Çalışmaya 11 yaşındaki pediatrik hasta dahil edildi. Hastanın eklem hareket açıklığı, kas kuvveti ve üst ekstremite becerisi değerlendirildi. Tüm değerlendirmeler tedavinin başlangıcında ve 12 haftalık tedavi sonrasında yapıldı. Tedavi sonrasında özellikle aktif dirsek fleksiyonunda 18°’lik artış olmak üzere dirsek ekstansiyonunda da iyileşmeler görüldü. El bileği fleksiyonunda 11° ve el bileği ekstansiyonunda 4°’lik artışlarla birlikte, ön kol supinasyon ve pronasyonu eklem hareket açıklıklarında minor kazanımlar gözlemlendi. Dirsek fleksiyonu, dirsek ekstansiyonu ve el bileği fleksiyonunun kas gücünde iyileşmeler görüldü. Üst ekstremite beceri skoru 3.96 puanlık artış gösterdi. Bu vaka raporu robot destekli tedavinin Erb-Duchenne Palsi’li hastalarda etkili bir rehabilitasyon yöntemi olabileceğini göstermiştir. Although various conservative rehabilitation approaches are widely used in rehabilitation clinics, the robot-assisted therapy, which has been used in recent years, has been used as a rehabilitation approach to improve upper extremity function in patients with neurological disorders. This study aimed to show the effect of robotassisted therapy on upper extremity motor function in a pediatric patient with Erb-Duchenne Palsy. An 11-year-old pediatric patient was included in the study. Range of motion, muscle strength and upper extremity ability of the patient were evaluated. All evaluations were made at the begining of treatment and at the end of 12-weeks of treatment. After treatment, improvements were obtained in elbow flexion and extension, particularly 18° increase was found in flexion. Minor gains were observed in the range of motion of the forearm supination and pronation, with an increase of 11° in wrist flexion and 4° in wrist extension. Remarkable improvements were observed in the strength of elbow flexion, elbow extension, and muscle strength of the wrist flexion. The upper extremity skill score increased by 3.96 points. This case report showed that robot-assisted therapy would be an effective rehabilitation method for in patients with Erb-Duchenne Palsy. Abstract:

Erb-Duchenne Palsi is a disease characterized by muscle weakness and loss of motor function due to injury in the upper brachial plexus. While various conservative rehabilitation approaches are widely used in rehabilitation clinics, robotic-backed treatment used in recent years has been used as an hopeful approach to improving the upper end function in patients with neurological disorders. This study was conducted in pediatric patients with Erb-Duchenne Palsi with the aim of showing the impact of robotic-backed treatment on the upper extremity motor function. The study included a 11-year-old pediatric patient. The patient’s joint movement clarity, muscle strength and upper end ability were assessed. All evaluations were made at the beginning of the treatment and after 12 weeks of treatment. After treatment, heals were observed in the extension of the rectum, with an increase of 18 degrees, especially in the active rectum fraction. Combined with 11° increase in handheld flektion and 4° increase in handheld extension, minor gains were observed in front arm suppression and pronation joint movement openings. It has been observed improvements in the muscle strength of the flektion of the wrist, the extension of the wrist and the flektion of the wrist. The top end skill score showed a 3.96-point increase. This case report showed that robotic-backed treatment could be an effective rehabilitation method in patients with Erb-Duchenne Palsi.
