AMAÇ: Gastrointestinal stromal tümörler (GİST) sindirim sisteminin en sık görülen mezenkimal tümörleri olup tüm gastrointestinal tümörlerin yaklaşık %3’ünü oluştururlar. Çalışmamızın amacı, GİST’lerin sindirim sisteminin malign epitelyal tümörleri ile birlikte görülme sıklığını araştırmak, birliktelik izlenen olgularda tümörlerin lokalizasyonları, histopatolojik ve immünohistokimyasal özelliklerini saptamak ve sadece GİST saptanan olgularla karşılaştırmaktır. YÖNTEMLER: Hastanemizin patoloji kliniğinde tanı alan 22 GİST olgusu çalışmaya dahil edildi. Bunlar arasından malign epitelyal tümör ile birliktelik gösterenler seçilerek ayrı bir grup oluşturuldu. Bu gruptaki malign epitelyal tümörlerde immünohistokimyasal olarak CD117 ve CD34 ekspresyonu araştırıldı. BULGULAR: Yirmi iki olgunun 5’inde (%22,7) GİST ile birlikte adenokarsinom saptandı. Bu olgularda GİST’lerin 3’ü mide, 2’si ise ince bağırsak yerleşimli idi. Bu gruptaki GİST’ler agresif davranış açısından düşük ya da çok düşük risk grubundaydı. GİST’lerle birlikte görülen adenokarsinomlar ise genellikle mide yerleşimli olup üç tanesi taşlı yüzük hücreli karsinomdu. Adenokarsinomlarda CD34 ve CD117 ile immünohistokimyasal olarak boyanma saptanmadı. SONUÇ: GİST’lerin sindirim sisteminin malign epitelyal tümörleri ile birlikteliği rastlantısal mı, yoksa farklı histopatolojik yapıya sahip bu tümörler gelişimleri sırasında ortak bir mekanizmayı kullanıyor olabilirler mi sorusunu cevaplamak için geniş serilerle yapılan çalışmalara ihtiyaç vardır.
Gastrointestinal stromal tumors (GST) are the most common mesenchimal tumors in the digestive system and account for approximately 3% of all gastrointestinal tumors. The purpose of our study is to investigate the frequency of GIS in the digestive system along with malignant epithelial tumors, to identify the localizations of the tumors, the histopatological and immune-histochemical properties in the observed events, and to compare it only with the GIS found. Methods: 22 GIST phenomena diagnosed in our hospital's pathological clinic were included in the study. A separate group was created by selecting those associated with malignant epithelial tumors. In this group of malignant epithelial tumors, CD117 and CD34 expressions were immunochemically studied. Discoveries: In 5 of twenty-two incidents (22.7%) adenocarcinoma was detected with GIST. In these cases, 3 of the GISTs were stomach and 2 of them were thin intestines. GITs in this group were in a low or very low risk group in terms of aggressive behavior. The adenocarcinomas that are seen along with GIS are usually stained in the stomach, and three of them were stone ring cell carcinomas. In adenocarcinomas, CD34 and CD117 have not been immune-histochemically painted. The question is whether the association of the digestive system with malignant epithelial tumors is random, or whether these tumors with a different histopathological structure may use a common mechanism during their development requires a wide range of studies to answer the question.
