So many results that aren't mine?

Yenidoğan Ünitelerinin İşleyişinde Ortaya Çıkan Etik Durumlar

Abstract:

Etik karar verme süreçlerinin layıkıyla gerçekleştirilmemesine neden olan yoğun sirkülasyon ve diğer olumsuz etkenler, etik farkındalık düzeyinde azalma veya körleşme gibi sıkıntılar ortaya çıkarabilmektedir. Aşırı iş yükü ve zaman sıkıntısı sağlık profesyonellerinin bireysel algoritmalar üreterek standart uygulamaları terk etmesi riskini yaratmaktadır. Yenidoğan yoğun bakım ünitelerinde çalışan sağlık profesyonelleri, etikle ilgili sorunlarla çok sık karşılaşmaktadır. En sık karşılaşılan sorunlar hekim görevlerinin hemşirelere devredilmesi, kıdemsiz çalışanlara “angarya” yüklenmesi ve uzun vardiyalar şeklinde çalışılması şeklindedir. En fazla önemsenen sorunlar ise aydınlatılmış onam formlarına imza alınmaması, ailelere açık ve ayrıntılı bilgi verilmemesi ve uzun vardiyalar şeklinde çalışılması olarak bildirilmektedir. Hem karşılaşma sıklığı hem de önemseme derecesi bakımından kural ihlalleri, değer çatışmalarının önüne geçmektedir. Bu derleme çalışması, Yenidoğan Yoğun Bakım Ünitelerinde ortaya çıkan etik durumları ve bu durumlarla karşılaşan sağlık profesyonellerinin yaşadığı sıkıntıları, karar verme süreçlerini ve çözümünde neler yapabileceklerini incelemek amacıyla yapılmıştır. Etik durumlar ve çözüm süreçleri hakkında, Yenidoğan Yoğun Bakım Ünitesi çalışanlarına düzenli aralıklarla hizmet içi eğitim verilmesi önerilmektedir.

Intense circulation and other negative factors that cause ethical decision-making processes not to be performed properly may cause problems such as decrease in the level of ethical awareness or blindness. Excessive workload and shortage of time creates the risk that health professionals will abandon standard practices by generating individual algorithms. Health professionals working in neonatal intensive care units often face ethical problems. The most common problems are transferring physician duties to nurses, loading "chore a to junior employees and working as long shifts. The most important problems are as lack of signed consent forms, lack of clear and detailed information for families and working as long shifts. In terms of both the frequency of the encounter and the degree of importance, violations of the rules prevent conflicts of value. This review was conducted to investigate the ethical issues that emerged in Neonatal Intensive Care Units and the problems experienced by health professionals who faced these situations, decision-making processes and what they could do to solve them. It is recommended that in-service training be given to the Neonatal Intensive Care Unit employees on a regular basis on ethical issues and resolution processes.
