Amaç: Nekrotizan fasiit (NF) hayatı ve ekstremiteyi tehdit edebilen, çok sık görülmeyen bir yumuşak doku enfeksiyonudur. NF, baş edilmesi en zor cerrahi enfeksiyonlardan biri olup, tabloya genellikle ileri sistemik toksisite de eşlik eder. Bu çalışmanın amacı alt ekstremite NF olgularında komorbid hastalıklarla birlikte serum laktat ve kreatinin seviyeleri ile, mortalite ve morbidite insidansı arasındaki ilişkiyi değerlendirmektir. Yöntem: Bu çalışmada, 87 alt ekstremite NF olgusunda tedavi şekli, serum laktat ve kreatinin seviyeleri ve semptomların ilk başlamasıyla, cerrahi uygulanan zaman arasında geçen süre ile amputasyon ve ortalama yaşam süresi arasındaki ilişki değerlendirilmiştir. Bulgular: Semptomların başlamasıyla cerrahi yapılma zamanı arasındaki ortalama süre 3.7 gün idi. Bu süre uzadığında amputasyon ve mortalite anlamlı şekilde arttığı saptanmıştır (p< 0.001). Serum kreatinin seviyesi 2 mg/dL’den fazla olan 12 hastada mortalite oranı % 66 olarak saptandı. Amputasyon/dezartikülasyon yapılan 14 hastanın 12’sinde (%85.7) ortalama serum laktat seviyesi 5.7 (5.1-8.7 arası) mmol/L, ortalama serum kreatinin seviyesi ise 1.92 (1.4-3.3 arası) mg/dL olarak saptandı. Başvuru esnasında serum kreatinin ve laktat seviyesinin yüksek olmasının, mortalite ve amputasyon görülme sıklığını istatistiksel olarak anlamlı şekilde arttırdığı saptanmıştır (p< 0.001). Sonuç: Bu çalışmanın sonuçlarına göre, alt ekstremite NF olgularında mortalite ve morbiditeyi arttıran başlıca faktörler; ileri yaş, geç başvuru, başvuru esnasında yüksek serum laktat ve kreatinin değerleri olarak saptanmıştır. Ayrıca bu parametreler, ilk başvuru esnasında hastalığın seyri açısından da fikir veren parametreler olarak akılda bulundurulmalıdır.
Purpose: Nekrotic phasicitis (NF) is a very unusual soft tissue infection that can threaten life and limbs. NF is one of the hardest surgical infections to deal with, and the table is often accompanied by advanced systemic toxicity. The aim of this study is to evaluate the relationship between the levels of serum lactate and creatine, as well as the complicated diseases in the lower limb NF cases, mortality and morbidity incidence. Method: This study assessed the relationship between the way of treatment, serum lactate and creatine levels and the first start of symptoms in 87 sub-extreme NF cases, the time spent between the surgery and the amputation and the average life-long. The average period between the start of symptoms and the time of surgery was 3.7 days. It was found that amputation and mortality increased significantly (p< 0.001). In 12 patients with serum creatine levels above 2 mg/dL, the mortality rate was 66%. In 12 of the 14 patients under amputation/desarticulation (85.7%) the average serum lactate level was 5.7 (5.1-8.7) mmol/L and the average serum creatine level was 1.92 (1.4-3.3) mg/dL. During the application, the high level of serum creatine and lactate has been found to statistically significantly increase mortality and amputation (p< 0.001). Results: According to the results of this study, the main factors that increase mortality and morbidity in lower end NF cases are identified as high levels of serum lactate and creatine during the application; advanced age, late application. Furthermore, these parameters should be taken into account as parameters that give an idea of the course of the disease during the first application.
