Abstract:

ÖZET Amaç: Kronik Obstrüktif Akciğer Hastalığı (KOAH) kardiyovasküler komorbiditeler, periferal kas disfonksiyonu, kilo kaybı, sistemik inflamasyon ve psikolojik problemler gibi birçok komorbidite ile beraber olabilir. Çalışmamızda, KOAH hastalık şiddeti ile fonksiyonel denge durumunu ve yaşam kalitesi üzerine etkileri arasındaki ilişkiyi incelemeyi amaçladık. Materyal ve Metod: Prospektif yapılan çalışmamızda hastalar “KOAH hastalar Küresel Girişim (GOLD) rehberine” göre A ile B grubu (hafif-orta) ve C ile D (ağır-çok ağır) grubu birleştirilerek iki grup oluşturuldu. Hastaların demografik verileri, solunum fonksiyon testleri, Modifiye Tıbbi Araştırma Konseyi Skalası (MRC), KOAH Değerlendirme Testi (CAT), St. George Solunum Anketi (SGRQ) ve Berg Denge Skalası uygulandı. Bulgular: Kırkbeş KOAH’lı olgu çalışmaya alındı. Denge skalası ile KOAH atak yılı, MRC ve CAT değerleri arasındaki fark anlamlıyken (sırasıyla p değerleri 0.016, 0.033 ve 0.0179), SGRQ aktivite, semptom ve total değerleri ile karşılaştırıldığında fark saptanmadı. Sonuçlar: Özellikle ileri evre hastalarda dinamik dengeyi değerlendirmek için Berg Denge Skalası kullanmak uygun olabilir. In this study, we aimed to investigate the relationship between COPD disease severity and balance. Materials and Methods: In our prospective study, patients were divided into two groups according to Global Initiative for Chronic Obstructive Lung Disease (GOLD) classification: group A and group B (mild to moderate) and group C and D (heavy to very heavy). Demographic data, pulmonary function tests, Modified Medical Research Council Scale (mMRC), COPD Evaluation Test (CAT), Saint George Respiratory Questionnaire (SGRQ), as well as the Berg Balance Scale were applied. Results: Forty-five patients with COPD were included in the study. As the stage progressed in COPD, worsening of balances was found. It may be appropriate to use the Berg Balance Scale to assess the dynamic balance, especially in advanced stage patients. While the difference between balance scale and COPD attack, MRC and CAT values were significant (p values 0.016, 0.033 and 0.0179, respectively), there was no difference between SGRQ activity, symptom and total values. Conclusions: It may be appropriate to use the Berg Balance Scale to assess the dynamic balance, especially in advanced stage patients. Chronic Obstructive Lung Disease (KOAH) may be associated with many complications such as cardiovascular complications, peripheral muscle dysfunction, weight loss, systemic inflammation and psychological problems. In our study, we aimed at studying the relationship between the severity of KOAH disease and functional balance and its effects on the quality of life. Materials and Methods: In our prospective study, two groups of patients were formed by combining group A and B (light-medium) and group C and D (heavy-very heavy) according to the "KOAH patients Global Initiative (GOLD) Guide". The demographic data of patients, respiratory function tests, Modified Medical Research Council Scale (MRC), KOAH Assessment Test (CAT), St. George's Respiratory Survey (SGRQ) and Berg Balance Scale were applied. The results: Fourteen KoAH facts have been taken to work. While the difference between the balance scale and the KOAH attack year, the MRC and CAT values (relevant p values; 0.016, 0.033 and 0.0179), the difference was not found when compared to the SGRQ activity, symptoms and total values. Results: It may be appropriate to use the Berg Balance Scale to evaluate the dynamic balance especially in advanced stages of patients.
