LUMBOSAKRAL BÖLGEDE ŞİŞLİK İLE BAŞVURAN ARTERİYOVENÖZ MALFORMASYON OLGUSU A Case of Arteriovenous Malformation Presenting With Swelling in The Lumbosacral Region

ÖZET Beyin ve sinir cerrahisi kliniğine lumbosakral bölgede kitle ile başvuran çocuklarda öncelikle meningosel kesesi başta olmak üzere konjenital spinal malformasyonlara ait patolojiler akla gelmektedir. Bununla birlikte bu bölgenin ayırıcı tanıları arasında lipom, konjenital vasküler malformasyonlar da sayılabilir. Bu sunumda lumbosakral bölgede ağrılı büyüme gösteren şişlik ile başvuran ve radyolojik görüntülerinde spinal disrafizme ait bulgu olmaması nedeniyle lipom ön tanısı ile ameliyat edilen ve arteriyovenöz malformasyon tanısı konulan 5 yaşında çocuk hasta literatür ışığında irdelenmektedir. Anahtar Kelimeler: Vasküler Malformasyon Arteriyovenöz Malformasyon Lipom Manyetik Rezonans Görüntüleme ABSTRACT Pathologies of congenital spinal malformations, especially the meningocele sac, come to mind in children who apply to the neurosurgery clinic with a mass in the lumbosacral region. However, the differential diagnoses of this region include lipoma and congenital vascular malformations. In this presentation, a 5-year-old pediatric patient who was admitted with a swelling on the lumbosacral region and who was operated for the diagnosis of lipoma due to absence of absence of spinal dysraphism findings in his radiological images but pathologically diagnosed as arteriovenous malformation in the light of the literature. Keywords: Vascular Malformation Arteriovenous Malformation Lipoma Magnetic Resonance Imaging GİRİŞ Vasküler anomaliler konjenital anormal vasküler gelişimli lezyonlardır. Önceleri vasküler doğum lekeleri olarak adlandırılmakla birlikte 1982 yılında Mulliken ve Glowacki tarafından histolojik, biyolojik davranış ve bu antitelerin klinik başvuru şekillerine göre sınıflandırılmışlardır (1). Vasküler anomaliler, vasküler tümörler ve vasküler malformasyonlar olmak üzere iki ana başlık altında toplanmaktadırlar. Vasküler malformasyonlar ise yavaş ve hızlı akımlı olarak iki alt gruba ayrılmaktadır. Arteriyovenöz malformasyonlar (AVM) hızlı akımlı vasküler malformasyonlar grubunu oluşturmaktadır. Gerek vasküler tümörler gerekse malformasyonlar vücudun her yerinde görülebilmektedirler (2). Arteriyovenöz malformasyonların orijini ve patogenezi hakkında çok az şey bilinmekte olup ergenlik ve travmanın bu lezyonların büyümesini tetiklediği bildirilmiştir (3). Bununla birlikte beyin ve sinir cerrahisi kliniğine lumbosakral bölgede yumuşak doku şişliği ile başvuran çocuk hastalarda ise alışılagelmiş deneyimlere paralel olarak ön planda meningosel kesesi gibi konjenital spinal malformasyonlar düşünülmektedir. Bu yazıda lumbosakral bölgede ağrılı ve ilerleyici büyüme gösteren şişlik ile başvuran ve arteriyovenöz malformasyon teşhisi konulan 5 yaşındaki bir erkek çocuk olgusu sunulmaktadır.

In children who apply to the brain and neurochirurgical clinic with the mass in the lumbosacral area, the pathologies of congenital spinal malformations, primarily the meningosal section, are thought. However, the distinctive diagnoses of this area may also include lipoma, congenital vascular malformations. In this presentation, a 5-year-old child, who applied with swelling showing painful growth in the lumbosacral area and has been operated with a pre-diagnosis of lipoma due to the absence of spinal disraphism in the radiological images and diagnosed with arteriovenous malformation, is in the light of the patient literature. Although they were first called vascular birth spots, they were classified by Mulliken and Glowacki in 1982 according to histological, biological behavior and the forms of clinical application of these antibodies (1). They are gathered under two main titles: vascular anomalies, vascular tumors and vascular malformations. Vascular malformations are slowly and quickly divided into two subgroups. Arteriovenous malformations (AVMs) form a group of rapidly flowing vascular malformations. If necessary vascular tumors, malformations can be seen everywhere in the body (2). Very little is known about the origin and pathogenesis of arteriovenous malformations, and it has been that adolescence and trauma have caused the growth of these lesions (3). However, in children who apply to the brain and nerve surgery clinic with soft tissue swelling in the lumbosacral area, congenital spinal malformations, such as the front-plan of the meningosal section, are considered parallel to the usual experiences. This article presents the fact of a 5-year-old boy who applies with a swelling that shows painful and progressive growth in the lumbosacral area and has been diagnosed with arteriovenous malformation.
