Amaç: Bu çalışmada hastanemizde bruselloz tanısı konulan çocuk hastaların sosyodemografik, klinik ve laboratuvar özelliklerinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmada Eylül 2009-Aralık 2014 tarihleri arasında, üçüncü basamak sağlık merkezi olan hastanemizde bruselloz tanısı konulan 16 yaş altındaki hastalar retrospektif olarak incelenmiştir. Bulgular: Çalışma süresi boyunca dosya verileri yeterli bulunan 94 çocuk çalışmaya dahil edilmiştir. Hastaların yaş ortalaması 8.85±3.69 yıldır ve %46.8’si erkek, kalan %53.2’si kızdır. Hastaların %75.5’i kırsal kesimde yaşamakta, %87.2’inin pastörize edilmemiş süt-peynir tüketimi, %69.1’inin ailesinde hayvancılıkla uğraşma, %30.8’inin ailesinde bruselloz geçirme öyküsü bulunmaktadır. Hastaların doktora ilk başvurusu, semptomları başladıktan ortanca 16 (aralık, 7-45) gün sonra olmuştur. Hastalarınsık başvuru yakınmaları; ateş (%88.2), artralji (%85.1) ve halsizlik (%80.8) iken;sık fizik inceleme bulguları ateş yüksekliği (%44.6), artrit (%12.7) ve hepatomegali (%8.5) olmuştur. Tanı konulan artritler monoartiküler özellikte vesık diz eklemi tutulumu mevcut idi. Başvuruda hastaların lökosit, C-reaktif protein, eritrosit sedimantasyon hızı ve transaminaz düzeyleri normal sınırlardaydı. Hastaların %13.5’indeaz bir seride sitopeni saptandı. Rose Bengal testi hastaların %91.5’inde, Brusella standart tüp aglütinasyon testi %88.2’sinde, Brusella Coombs aglütinasyon testiyse %94.6’sında pozitifti. Hastaların %7.1’inde kan kültüründe Brucella spp. üremesi oldu. İki hastada hepatit ve 1 hastada meningoensefalit saptandı. Yedi hastada bir yıl içerisinde relaps veya reenfeksiyon gelişirken çalışmada mortalite gözlenmedi. Sonuç: Brusellozis ülkemizde Doğu Anadolu Bölgesi’nde sık görülmektedir. Özgül olmayan klinik ve laboratuvar bulguları nedeniyle özellikle endemik bölgelerde sürekli akılda bulundurulması gereken bir hastalıktır. Antibiyotik tedavisini uygun sürede verilmesiyle komplikasyonlar ve tedavi başarısızlığı riski azalacaktır.
Purpose: This study aims to evaluate the sociodemographic, clinical and laboratory characteristics of children diagnosed with brusellosis in our hospital. In the study, between September 2009 and December 2014, patients under the age of 16 who were diagnosed with bruselosis in our hospital, the third stage health center, were retrospective examinations. Results: 94 children with sufficient file data during the work period were included in the study. The average age of patients is 8.85±3.69 years and 46.8 percent are male and 53.2 percent are girls. 75.5 percent of patients live in rural areas, 87.2 percent have a history of unpasteurized milk and cheese consumption, 69.1 percent of their families have a history of livestock and 30.8 percent have a history of brusellosis in their families. The patient's first appeal to the doctor was about 16 (December, 7-45) days after the symptoms began. Patients frequently contacted with fever (%88.2), arthralgia (%85.1) and discomfort (%80.8) while frequent physical examination findings were high fever (%44.6), arthritis (%12.7) and hepatomegaly (%8.5). The diagnosed arthritis was monoarthetic characteristic of the arthritis of the joint joint. In the application, the patient’s leukocyte, C-reactive protein, erythrosite sedimentation rate and transaminase levels were at normal limits. At least 13.5 percent of patients were diagnosed with a series of cytopenia. The Rose Bengal test was positive in 91.5 percent of patients, the Brusella standard tube aglutination test in 88.2 percent, and the Brusella Coombs aglutination test in 94.6 percent. In 7.1 percent of patients, Brucella spp is in blood culture. It has grown. There were two patients with hepatitis and one with meningoencephalitis. Seven patients had no mortality in the study while recurrence or reinfection developed within a year. The result: Brussels is common in the Eastern Anatolian region in our country. It is a disease that should be kept in mind, especially in endemic areas, due to non-specific clinical and laboratory findings. If antibiotic treatment is given timely, the risk of complications and treatment failure will be reduced.
