Aday Doktorların Mezuniyet Öncesi Dönemde Temel Hekimlik Uygulamalarındaki Yeterlik Düzeyi

Abstract:

Giriş: Yetkinlik bir kişinin görevleri doğru ve ustaca yapabilmesi için sahip olması gereken bilgi, beceri, yetenek, davranışlar ya da kapasitedir. Yetkinlik, tıp öğrencileri için önemli kriterlerdendir. UÇEP’in temel bileşenlerden birisi “Temel Hekimlik Uygulamaları”dır. Bu alanların dört dereceli öğrenme seviyesi bulunmaktadır. Bu çalışmanın amacı COVID-19 pandemisi nedeniyle 9 ay gibi bir süre eğitim alan aday doktorların “temel hekimlik uygulamaları” başlıklarında yeterlik algılarını değerlendirmektir. Gereç ve Yöntem: Akdeniz Üniversitesi Tıp Fakültesi’nde 2019-2020 Eğitim-Öğretim Yılında 9 aylık aday doktorluk stajını tamamlayan öğrenciler çalışma örneklemini oluşturmaktadır. Öğrencilere UÇEP-2020’de yer alan temel hekimlik uygulamalarındaki yeterlik düzey algılarını belirtmeleri istenmiştir. Belirttikleri yeterlik düzeyleri stajlarını kliniklerde ya da uzaktan eğitim yöntemi ile almalarına göre karşılaştırılmıştır. Analizlerde tanımlayıcı tabloların yanı sıra ki kare testi kullanılmıştır. Bulgular: Anket formunu 262 öğrenci doldurmuştur. Adli Tıp, Psikiyatri ile Kadın Hastalıkları ve Doğum gibi alanlara spesifik becerilerde stajı alan ve almayan öğrencilerin yeterlik algılarında farklılık olduğu izlenmiştir (p<0,05). Öğrencilerin tamamı tarafından kazanılmış olması beklenen UÇEP’te yer alan becerilerin bütününe bakıldığında yaklaşık %25 yapabilme durumlarında kayıplar olduğu görülmektedir. Sonuç: Öğrencilerin UÇEP-2020’de tanımlanmış temel hekimlik becerilerine göre birinci basamak sağlık hizmetleri için yeterince hazır olmadıkları söylenebilir. Öğrencilerin yetersiz olduğu becerilerin eğitim programının hangi süreçlerinde yer aldığı ve yetersizlik nedenlerinin araştırılması gerekir. Öğrenciler genel olarak becerilerde kendilerini yeterli hissetmekle birlikte bazı alanlarda eğitim sistemi, çekirdek eğitim programı ve sağlık sistemi ile ilgili düzenlemelerin yapılması gerekliliği dikkat çekmektedir.

Introduction: Competence is the knowledge, skills, abilities, behaviors or capabilities that a person needs to have in order to perform his tasks properly and skillfully. Competence is an important criterion for medical students. One of the main components of UÇEP is "Basic Medical Applications". These fields have a four-degree learning level. The aim of this study is to evaluate the qualification perceptions of candidate doctors who have been trained for a period of as 9 months due to COVID-19 pandemic in the "Basic Medical Practices" titles. Tools and Methods: Students who complete the 9 months candidate doctoral internship in the 2019-2020 Education-Teaching Year at the Mediterranean University School of Medicine form the study sample. Students are asked to indicate their qualification level perceptions in the basic medical practices included in the UÇEP-2020. The qualification levels they indicate are compared by taking their internships in clinics or through the method of distance learning. In addition to the identification tables in the analysis, the square test was used. The survey form was filled with 262 students. In the fields such as Judicial Medicine, Psychiatry and Women’s Diseases and Birth, there has been a difference in the perception of competence for students who have and have not been trained in specific skills (p<0,05). If you look at all the skills included in the UÇEP, which is expected to be gained by all students, there are 25% loss in cases of ability to do so. Result: Students can be said that they are not ready enough for first-level health care according to the basic medical skills defined in the UCHEP-2020. It is necessary to investigate in which processes the educational program involves the skills in which students are inadequate and the causes of disability. While students generally feel sufficient in skills, they draw attention to the need to make regulations related to the educational system, core training program and health system in some areas.

Background: Competency is the knowledge, skills, ability, and behaviors or capacities that a person possesses to perform tasks correctly and skillfully. Competency is an important criterion for medical students. “Basic Medical Practices” (BMP) are among the core components of the National Core Education Program (NCEP). These domains have four levels of learning. This study aims to evaluate the competencies in “BMP” domains of final year medical students’ who could complete almost 9-month of education on site before the COVID-19 pandemic. Materials and Method: The final year medical students who completed 9 months of internship during the 2019-2020 academic year constitute the sample group at Akdeniz University Faculty of Medicine. The students were asked to self evaluate their competency levels of the BMP stated in NCEP-2020. Self-evaluated competency levels are compared based on receiving in-clinic versus online education. For the analysis of the study chi-squared test is used. Results: 262 students filled out the questionnaire. The difference between self-evaluated competency levels of specific practices pertaining to Forensic Medicine, Psychiatry, and Obstetrics and Gynecology specialties were found to be statistically significant between the students who completed the clerkships and those who did not (p<0,05). When the total of the skills in NCEP-2020 is reviewed, it is seen that there is a loss of being able to do after graduation about 25%. Conclusion: The study showed that some of the medical students were not sufficient for primary health care services according to the fundamental medical skills defined in the NCEP-2020. It is necessary to investigate which skills are inadequate and the reasons of the inadequacy. Although the students are feeling confident in many skills, it is noted that making adjustments in the education system, the core education program, and the healthcare system is necessary.
