Mental Motor Retarde Çocuklarda Hepatit A ve B Sıklığı

Amaç: Motor ve mental fonksiyon kaybı olan bakıma muhtaç çocuklar hepatit A virus (HAV), hepatit B virus (HBV) enfeksiyonu gibi bulaşıcı hastalıklar yönünden risk altındadır. Literatürde mental retardeli çocuklarda HAV ve HBV enfeksiyon sıklığı ile ilgili az sayıda çalışma bulunmaktadır. Ülkemizde bu konudaki tek çalışma spastik çocuklara özel hizmet veren bir hastanede yapılmış olup sadece evde bakılan mental motor retardeli (MMR) çocuklarda benzer çalışmaya rastlanmadı. Bu nedenle evde bakıma muhtaç mental motor retardeli çocuklarda anti HAV IgG, HBs Ag, anti- HBc total ve anti- HBs sıklığını araştırdık. Yöntemler: Çocuk Gastroenteroloji polikliniğine Haziran 2015- Aralık 2016 tarihleri arasında beslenme düzenlenmesi nedeni ile getirilen, motor ve mental fonksiyonlarını kaybeden 95 olgu alındı. Tüm olguların HBsAg, anti-HBs, anti- HBc IgG ve anti HAV düzeyleri “Chemiluminescent Assay ELISA” (COBAS TaqMan 48, Roche Diagnostics, Pleasanton, CA, USA) yöntemi ile değerlendirildi. Bulgular: Doksan beş olgunun %47’si (45 olgu) kız, %53’i (50 olgu) erkek olup yaş ortalaması 9±1,7 yaş (4-17 yaş) idi. Olguların %63’ünde anti-HAV IgG pozitif, %37’sinde negatif bulundu. Anti-HAV IgG pozitif olguların %13’ü hepatit A aşısı ile aşılanmış idi. Geri kalan %87’si ise aşısız idi. Olguların %74’ünde anti-HBs pozitif, %26’sında anti-HBs negatif saptandı. Anti-HBs pozitif olanların %81,4’ü aşılı, geri kalan %18,6’sı aşılama öncesi doğan çocuklar idi. Yaşlara göre hepatit A ve B seroprevelans dağılımı; 4-7 yaşında %23, 8-12 yaşında %32 13-17 yaşında ise %45 saptandı. Hepatit B seroprevalansı ise 4-7 yaşında %21, 8-12 yaşında %46 13-17 yaşında ise %33 saptandı. Sonuç: Sonuç olarak çalışmamızdaki hepatit A ve B seroprevalans oranlarını, bakımevlerinde yapılan çalışmaların aksine sağlıklı çocuklardakinden farklı bulamadık. Bunu da olgularımızın tümünün bakımevlerinde değil aile ortamında bakılmaları yanı sıra ailelerin bu çocuklarda sağlıklı çocuklara uygulanan aşılama programına uymaları ile açıklayabiliriz.

Influence of hepatitis A and B in children

Target: Children with motor and mental function loss are at risk of infectious diseases such as hepatitis A virus (HAV), hepatitis B virus (HBV). There are few studies in literature on the frequency of HAV and HBV infections in children with mental retardation. The only study on this issue in our country was done in a hospital that provides special services to spastic children, and the same work was not found in children with mental motor retardation (MMR) care only at home. Therefore, we studied the frequency of anti-HAV IgG, HBs Ag, anti-HBc total and anti-HBs in children with mental motor retardation needed at home. Methods: 95 cases of loss of motor and mental functions, brought to the child gastroenterology clinic between June 2015 and December 2016, due to nutrition regulation. The HBsAg, anti-HBs, anti-HBc IgG and anti-HAV levels of all incidents were evaluated by the "Chemiluminescent Assay ELISA" (COBAS TaqMan 48, Roche Diagnostics, Pleasanton, CA, USA) method. Results: 47% of the total five matures (45 cases) were girls, 53% (50 cases) were men and the average age was 9±1.7 years (4-17 years). In 63% of cases, anti-HAV IgG was positive and in 37% negative. 13% of anti-HAV IgG positives were vaccinated with a hepatitis A vaccine. The remaining 87% were non-vaccinated. In 74% of cases anti-HBs were positive and in 26% anti-HBs negative. 81.4% of those who were anti-HBs positive were vaccinated, while the remaining 18.6% were pre-vaccinated children. Hepatitis A and B seroprevelans are distributed by age; 23% in the ages of 4-7 years, 32% in the ages of 8-12 years and 45% in the ages of 13-17 years. Seroprevalence of hepatitis B was diagnosed at 4-7 years of age 21%, at 8-12 years of age 46% and at 13-17 years of age 33%. As a result, the rates of seroprevalence of hepatitis A and B in our study were not found different from healthy children, unlike the studies in the caregivers. This can be explained by the fact that all of our facts are taken care of in the family environment, and the families follow the vaccination program applied to healthy children in these children.
