AKCİĞER KANSERİ NEDENİYLE AMELİYAT EDİLEN HASTALARDA OPERASYONEL ANESTEZİ SÜRECİNİN ANALİZİ

Amaç: Bu çalışmada, primer akciğer kanseri nedeniyle opere edilen hastalarda cerrahi rezeksiyon tipine göre anestezi tarafından uygulanan yaklaşımlar ve süreleri retrospektif olarak incelenmiş, farklılık olup olmadığı araştırılmıştır. Yöntem: Kliniğimizde Ocak-Eylül 2004 döneminde primer akciğer kanseri nedeniyle opere edilen 42 hastanın anestezi kayıtları analiz edildi. Çalışmaya alınan olguların cinsiyet, tümör tipi, tümör evresi, çift lümenli endotrakeal tüp lokalizasyonu, torakotomi yönü, yapılan cerrahi rezeksiyon  türü,  anestezi  süresi,  tek  akciğer  ventilasyon  süresi,  sıvı,kolloid mayi ve kan replasmanı incelenip ortalamaları alındı. Bulgular: Lobektomi yapılan hastalarda cerrahi anestezi süresi ortalama 235±84 dakika olup pnömonektomilerde ise cerrahi anestezi daha uzun olarak ortalama 260±75 dakika sürmüştür. Her iki rezeksiyon tipinin ameliyat süreleri arasında istatistiksel fark saptanmamıştır (p>0.05). Tek akciğer ventilasyon süresi karşılaştırıldığında da benzer şekilde lobektomi (203±79 dakika) yapılanlarla pnömonektomi (242±75 dakika) yapılanlar arasında istatistiksel fark bulunmamıştır (p>0.05). Neoadjuvan tedavi alan hastaların ortalama ameliyat süresi 228.75±87.55 dakikadır, primer olarak cerrahi tedavi gören hastalarda ise 259.29±66.48 dakika cerrahi süresi gerekmiştir, bu değerler de istatistiksel olarak karşılaştırıldığında fark bulunmamıştır (p>0.05 (p=0.317)). Sonuç: Bu çalışmada rezeksiyon tipinin anestezi ve cerrahi süresi üzerinde etkisi olup olmadığı incelendiğinde istatistiksel fark olmadığı ortaya konmuştur. Methods: The anesthesia reports of 42 patients that had undergone surgery for lung cancer from January 2004 to September 2004 were analyzed. The patients’ gender, tumor cytology, stage of tumor localization of the double lumen endotracheal tube, thoracotomy side, type of resection, time of anesthesia, single lung ventilation time, fluid, colloid fluid and blood transfusion were analyzed. Results: Patients who underwent lobectomy had a surgical anesthesia time of 235±84 minutes, whereas in patients who underwent pneumonectomy the mean value was 260±75 minutes. The difference between the two types of resection was not statistically significant (p > 0.05). Similarly, single lung ventilation time did not differ significantly (p > 0.05) between patients who underwent lobectomy (203 ± 79) and patients who underwent pneumonectomy (242 ± 75 minutes). The mean operation time of patients who received neoadjuvant chemotherapy was 228 ± 87.55 minutes, while patients who underwent primary surgical treatment had a mean value of 259 ± 66.48 minutes. There was no statistically significant difference between these two values (p> 0.05, (p= 0.317)). Conclusion: No statistically significant difference was found between type of resection, anesthesia, and surgical duration.Key Words: Lung cancer, Anesthesia, Peroperative period, Analysis

Purpose: In this study, patients operated due to primary lung cancer have been examined retrospectively the approaches and durations applied by anesthesia according to the type of surgical resection, whether there are differences. Method: In our clinic, the records of anesthesia of 42 patients operated due to primary lung cancer in the period January-September 2004 were analyzed. The studies of the incidents of gender, type of tumor, stage of tumor, double-luminal endotrakeal tube localization, toracotomy direction, type of surgical resection, anesthesia time, single lung ventilation time, liquid,colloid may and blood replacement were studied and taken. In patients with lobectomy, an anesthesia duration is an average of 235 ± 84 minutes, while in pneumonectomy, an anesthesia duration is an average of 260 ± 75 minutes. There is no statistical difference between the duration of both types of recession (p>0.05). There was no statistical difference between lobectomy (203 ± 79 minutes) and pneumonectomy (242 ± 75 minutes) compared to single lung ventilation time (p> 0.05). The average duration of surgery for patients receiving neoadjuvan treatment is 228.75±87.55 minutes, while the duration of surgery for patients receiving primary surgery is 259.29±66.48 minutes, and these values have not been distinguished when compared statistically (p>0.05 (p=0.317)). The study found that there was no statistical difference when it was studied whether the type of resection affects anesthesia and the duration of the surgery.
