Psikiyatrik bozukluklar içinde şizofreni yaygınlığının ve antipsikotik ilaç kullanımının gitgide arttığı görülmektedir. Bu çalışmada ayaktan tedavi amacıyla bir üniversite hastanesi psikiyatri polikliniğine başvuran hastalara yazılan reçetelerin şizofreni tanısı ve antipsikotik ilaç kullanımı açısından değerlendirilmesi amaçlanmıştır. Araştırma verileri, hastane başhekimliği ve etik kurul izni ile hastane hasta veri tabanı (ENLİL) sisteminden alınarak, hastalara yazılmış olan 17500 reçete üzerinden geriye dönük olarak tanımlayıcı bir reçete değerlendirmesi yapılmıştır. İstatistiksel analiz için SPSS 21.0 programı kullanılmış, alınan veriler direkt olarak bu program aracılığıyla veri tabanına işlenmiş ve değerlendirilmiştir. Hastanenin psikiyatri polikliniğine ayaktan başvuran DSM-V’e göre ilk tanısı şizofreni olan yetişkin hastalar ve şizofreni tanısı alıp almamasına bakılmaksızın antipsikotik reçetelenen yetişkin hastalar çalışmaya dahil edilirken, bu kriterleri karşılamayan hastalar ve epilepsi hastaları çalışmaya dahil edilmemiştir. Değerlendirilen reçetelerde ilk tanısı şizofreni olan toplam 3615 reçete vardır. Bu reçetelerdeki hastaların %54’ü erkek, %46’sı kadındır. Hastalardaki ortalama yaş 42.1 yıldır. Hastaların yaş gruplarına göre şizofreni tanılı reçetelerin oranı; 18-25 yaş arası %11.6, 26-35 yaş arası %22.7, 36-45 yaş arası %25.4, 46-55 yaş arası %23.9 ve 56 yaş ve üzeri grubunda %16.4’dır. Şizofreni tanısı almış hastalaraçok reçetelenen etken maddeler içinde klozapin (%13.2), ketiapin (%11.9) ve risperidon (%10.3) üst sıralardadır. Şizofreni tanısı alıp almamasına bakılmaksızın 6521 tane reçeteye antipsikotik ilaç yazılmakla birlikte bu reçetelerdeki hastaların %45.5’i erkek, %54.5’i kadındır. Şizofreni tanısı alıp almamasına bakılmaksızın antipsikotik ilaç yazılan reçetelerde tüm hastalaraçok reçetelenen antipsikotikler sırasıyla ketiapin (%25.9), aripiprazol (%21.6), risperidon (%11.8), olanzapin (%11.1) ve amisülpirid (%9.3)’dir. Erkek hastalaraçok reçetelenen antipsikotik ketiapin (%24.4) iken, kadın hastalara aripiprazol (%27.3)’dür. 18-25 yaş, 26-35 yaş ve 36-45 yaş arasınaçok aripiprazol; 46-55 yaş ve 56 yaş ve üzeri gruba iseçok ketiapin reçetelenmektedir. Reçetelerdeki ilaç yazımı seçiminde doktorların ilaç tercihi, hasta uyumu, hastaların cinsiyeti, yaş aralığı, ilaçların olası yan etkileri ve hastalıkların tanı-tedavi şekli gibi faktörlerin rol oynaması ilaç reçetelenmesindeki çeşitlilik ve farklılıkları arttırmaktadır.
In psychiatric disorders, the prevalence of schizophrenia and the use of antipsychotic drugs is increasing. This study aims to evaluate the prescriptions written to patients applying to a university hospital psychiatric clinic for the purpose of foot treatment in terms of diagnosis of schizophrenia and the use of antipsychotic drugs. The research data, the hospital doctors and the ethical board’s permission and the hospital patient database (ENLIL) system have been taken, and a recurring prescription assessment has been made over 17500 prescriptions written to patients. For statistical analysis, the SPSS 21.0 program was used, the received data was processed and evaluated directly through this program into the database. According to the DSM-V, the first diagnosis of adult patients with schizophrenia and adult patients prescribed with antipsychotics regardless of whether they are diagnosed with schizophrenia is included in the study, while patients and epilepsy patients who do not meet these criteria are not included in the study. There are 3615 prescriptions with the first diagnosis of schizophrenia. 54% of patients in these prescriptions are men and 46% are women. The average age of patients is 42.1 years. The ratio of prescriptions diagnosed with schizophrenia according to the age groups of patients is 11.6% between 18 and 25 years of age, 22.7% between 26 and 35 years of age, 25.4% between 36 and 45 years of age, 23.9% between 46 and 55 years of age and 16.4% in the age group of 56 and older. In patients diagnosed with schizophrenia, the most prescribed active substances include klozapine (13.2%), ketiapine (11%). 9) and risperidon (10.3%) are in the top ranges. 6521 prescriptions of antipsychotic drugs were prescribed, regardless of whether they were diagnosed with schizophrenia or not, but 45.5% of these prescriptions were male and 54.5% female. Regardless of whether you are diagnosed with schizophrenia or not, antipsychotic drugs are prescribed to all patients, the most prescribed antipsychotic drugs are ketiapine (25.9%, aripiprazole (21.6%, risperidon (11.8%, olanzapin (11.1%) and amisulpiride (9.3%, respectively). The most prescribed antipsychotic ketiapine is 24.4%, while aripiprazole is 27.3 for female patients. A lot of aripiprazole is prescribed in the ages of 18-25 years, 26-35 years and 36-45 years; a lot of ketiapine is prescribed in the ages of 46-55 years and 56 years and older. The role of factors such as the doctor's prescription in prescriptions, patient conformity, patient's gender, age range, possible side effects of medications and the diagnosis and treatment of diseases increases the diversity and differences in the prescription of medications.
