Amaç: Dokuz Eylül Üniversitesi (DEÜ) Hastanesi psikiyatri polikliniğine başvuranların birinci basamak kullanım durumu ve bunu etkileyen etmenlerin incelenmesi amaçlanmıştır. Gereç ve yöntem: Kesitsel bir araştırmadır. Bağımlı değişken birinci basamak kullanım durumu ve bağımsız değişkenler ise; cinsiyet, yaş, meslek, medeni durum, sosyal güvence durumu, öğrenim durumu, yaşadığı yer, gelir seviyesidir. DEÜ Hastanesi Psikiyatri polikliniğine 1-31 Ocak 2016 döneminde başvuranlarla yüz yüze görüşülerek, hazırlanan anket formu uygulanmıştır. Ki-kare testi ve Fisher’in kesin testi kullanılmıştır. Bulgular: Katılımcıların %13,7’si psikiyatrik sorunları nedeniyle aile sağlığı merkezine başvurmuştur. Psikiyatrik sorunları nedeniyleaz bir kez birinci basamağı kullanan hastaların (n=25) %40,0’ı doğrudan hastaneye sevk edilmiş, %32,0’ına ise ilaç yazılıp sonra sevk edilmiş, %28,0’ına sadece ilaç yazılmıştır. Katılımcıların sosyoekonomik özelliklerine göre psikiyatrik sorunları nedeniyle birinci basamak sağlık hizmeti kullanımı incelendiğinde; evli olmayanlarda evli olanlara göre kullanım anlamlı olarak yüksektir (p=0,043). Yaş, cinsiyet, meslek, sosyal güvence, öğrenim durumu, yaşadığı yer, gelir seviyesi ile birinci basamağı kullanım arasında anlamlı bir ilişki saptanmamıştır. Sonuç: Psikiyatrik sorunlar nedeniyle Aile Sağlığı Merkezi’ne başvuru oldukça düşüktür. Bu düşüklüğün sebepleri; psikiyatrik hastalıkların sadece uzman hekim tarafından tedavi edilebileceği, pratisyen hekime olan güvensizlik ve kısıtlı zaman ayrılması olarak belirtilmiştir. ASM hizmetlerinin içerisinde ruh sağlığı hizmetlerinin de yer aldığı konusunda toplum bilgilendirilmelidir.
Purpose: The 9th September University (DEU) Hospital's psychiatric clinic is intended to study the first stage use status and the factors affecting it. Method and method: It is an extensive study. Dependent variable is the first stage use status and independent variables; gender, age, profession, civil status, social security status, state of learning, where it lives, income level. The DEU Hospital Psychiatric Clinic in the period 1-31 January 2016 faced-faced with the applicants, the prepared survey form was applied. The Ki-Care test and Fisher's exact test were used. Results: 13.7 percent of participants appealed to a family health center due to psychiatric problems. As a result of psychiatric problems, 40.0 percent of patients using the first stage (n=25) were sent directly to the hospital, 32.0 percent were prescribed and subsequently transmitted, 28.0 percent were prescribed. According to the socio-economic characteristics of the participants, when the use of first-level health care due to psychiatric problems is studied, the use is significantly higher compared to those who are not married (p=0,043). There is no meaningful relationship between age, gender, occupation, social security, state of education, the place where he lives, the level of income and the first step of use. Result: The application to the Family Health Center is quite low due to psychiatric problems. The causes of this decline are indicated as: psychiatric diseases can only be treated by a specialist physician, uncertainty and limited time distribution to the practitioner physician. The community should be informed that the mental health services are also included in the ASM services.
