Goal: The Gaitada Hidden Blood (GGK) test is used in the scan of colorectal cancer. The GGK test is positive but the need for the endoscopy of the upper gastrointestinal system (GIS) in patients with normal colonoscopy is discussed. The purpose of this study is to investigate which group of patients with colonoscopy is normal in the results of the simultaneous lower and upper GIS endoscopy performed to patients with GGK test positive, which group of patients should be recommended for the upper GIS endoscopy. Instruments and Methods: Patients with lower and upper GIS endoscopy were scaned retrospectively in the same session due to GGK test positivity between January 2017 and March 2020. The study included 379 people with a normal colonoscopy of 626 patients. The demographic characteristics of patients, endoscopic findings, pathological outcomes and hemoglobin values were studied from hospital records. Results: The GGK test was positive and 139 (36.6%) of 379 patients with a normal colonoscopy were male and an average age of 57.7 (18-88). H. pylori was positive in 46% of patients, negative in 35% and untreated in 19% of patients. Gastric polyp was detected in 6 patients and in 4 of these patients had H. pylori positive. 5 patients were diagnosed with malignity of the stomach. 13% of benign pathologies were polyp, varicose, ulcerative, and 46% of the diseases that require follow-up such as H. pylori positive. Although the low hemoglobin was not statistically meaningful, it was lower than the other patients diagnosed in the tumor group (8.4 g/dL vs. 11.8 g/dL). Result: In our study, at the top GIS endoscopy, 13% of patients were identified with benign pathologies that require tracking, such as polyp, varicose, ulcerative and 46% H. pylori positive. Therefore, the GGK test should be recommended for patients who are positive and who do not detect pathology in their colonoscopy to perform the upper GIS endoscopy.
