Kemik iliği fibrozis ve anjiogenezisinin hematopoetik kök hücre engraftmanı üzerine etkisi

Amaç: Birçok faktör hematopoetik kök hücre (HKH) engraftmanını etkiler. Bu çalışmada hematolojik kanseri olan hastalarda kemik iliğindeki myelofibrozis ve anjiogenezisin engraftman üzerine etkisi araştırılmıştır. Gereç ve Yöntem: Otolog kök hücre nakli yapılan 34 hasta (20 erkek, 14 kadın) verileri değerlendirildi. Nakilden önceki son kemik iliğindeki fibrozis 0-3 arasında derecelendirildi, anjiogenezis stereolojik metod ile ölçüldü. Hastalar anjiogenezis parametrelerinin yoğunluğuna göre de iki gruba ayrıldı. Bulgular: Hastaların çoğunluğu (%73.5) multipl myelom idi ve yarısında fibrozis saptandı. On bir hasta derece 1, 6 hasta derece 2 fibrozise sahipti. Trombosit ve eritrosit engraftman günleri açısından derece 2 fibrozisi olan grupla fibrozis saptanmayan grup arasında istatistiksel anlamlı farklılık vardı. Derece 1 ve 2 fibrozis gruplarında fibrozisi olmayanlara göre VSD ve NVES düzeyleri anlamlı olarak yüksekti. Toplam yaşam derece 2 fibrozisi olan grupta daha düşük olmakla birlikte istatistiksel anlamlılık yoktu. Sonuç: Kemik iliği myelofibrozisinin bağımsız bir risk faktörü olduğu saptanmıştır. Bunun toplam yaşamı etkilemeden otolog nakil sürecinde trombosit ve eritrosit engraftmanı üzerine negatif etkisi olabilir. Fibrozisin derecesi ile doğru orantılı olarak artmış anjiogenezis arasındaki ilişkiyi açıklamak için ileri çalışmalar gereklidir.

The effect of bone marrow fibrosis and anjiogenesis on hematopoetic root cell injection

Abstract:

Purpose: Many factors affect the hematopoetic root cell (HKH) inhibition. In this study, the effects of myelofibrosis and anjiogenesis in the bone marrow in patients with hematological cancer were studied. Tools and Methods: The data of 34 patients (20 men, 14 women) were evaluated by the autologist root cell transplant. Fibrosis in the last bone marrow before the transplant was scored between 0-3, and angiogenesis was measured by stereological method. Patients were also divided into two groups according to the intensity of the anjiogenesis parameters. The majority of patients (73.5%) had multiple myeloma and half of them had fibrosis. One in ten patients had a degree of 1, 6 patients had a degree of 2 fibrosis. There was a statistically significant difference between the group with a degree 2 fibrosis and the group with a non-detected fibrosis in terms of the days of thrombocytes and erythrosis. The levels of VSD and NVES were significantly higher compared to those with non-fibrosis in grade 1 and 2 fibrosis groups. Although the group with a total life degree of 2 fibrosis was lower, there was no statistical significance. The result: The bone marrow myelofibrosis has been found to be an independent risk factor. This may have a negative effect on the thrombocytes and erythrosites in the process of autologist transplant without affecting the overall life. Advanced studies are required to explain the relationship between the level of fibrosis and correctly proportional increased angiogenesis.
