Plevral Efüzyonlarda Minimal İnvaziv Yaklaşım: Küçük Çaplı Plevral Drenaj Kateteri (Pleuracan®)

ÖZET Amaç: Küçük çaplı plevral drenaj kateteri (KPK) kullandığımız hastalarınyaş, cinsiyet, etyolojik faktör, kateter işleminin uygulandığı taraf ve drenajın etkinliğini retrospektif olarak araştırmayı planladık. Materyal ve Metot: Ocak 2015-Aralık 2017 tarihleri arasında KPK uyguladığımız 185 hastanın dosyaları retrospektif olarak incelendi. Plevral efüzyonlu (PE) hastaların tanıları, sıvının hangi hemitoraksta toplandığı, kateter komplikasyonları not edildi.Bulgular: Yüz seksen beş hastanın 107 (%57,9)’si erkek, 78 (%42,1)’i kadın olup yaş ortalamaları 52,3 (17–85)/yıl idi. Hastaların PE sitolojilerinin 12 (%6,5)’si malign 173 (%93,5)’ü benign olarak saptandı. Hasta kaynaklı nedenlerden dolayı üç hastada (%1,6) kateterin çıkması üzerine ikinci bir kateter takıldı. Yedi (%3,7) hastada kateterin intraplevral alanda king yapması nedeni ile kateter pozisyonu değiştirildi. Ekspansiyon kusuru tespit edilen 4 (%2,1) hastada tüp torakostomi işlemine geçildi. Sonuç: Günümüzde PE tedavisinde düşük maliyetli, kullanışlı, ideal yöntem arayışları devam etmektedir. Son yıllarda özellikle malign PE’da kalıcı kateterler tercih edilmektedir. En sık kullanılan tünelli plevral kateterlerdir. KPK acil servisde, yoğun bakım ünitesinde, klinikte yatak başında uygulanabilen minimal invazif bir cerrahi işlemdir.

Abstract:

Subject Purpose: We have planned to retrospective research the age, gender, etiological factor, the side where the catheter process is applied and the effectiveness of the drainage of the patients we use. Materials and Methods: From January 2015 to December 2017, the files of 185 patients we applied KPK were retrospective examined. Diagnoses of patients with pleural fusion (PE) were noted, catheter complications were noted, in which the fluid was gathered in the hemorrhoids.The findings: One hundred and seventy-five patients were 107 (57.9%) male, 78 (42.1%) female, and the average age was 52.3 (17-85)/year. 12 (6.5%) of patients' PE cytologies were malignant; 173 (93,5%) were benign. Three patients (1.6%) were infected with a second catheter after the catheter was released due to the cause of the patient. In seven (3.7%) patients, the position of the catheter was changed due to the fact that the catheter was king in the intrapleural area. 4 (2.1%) patients with expansion defect were diagnosed with tube toracostomy. The result: Today, the search for low-cost, convenient, ideal methods in PE treatment continues. In recent years, permanent cateters are preferred especially in malign PE. The most commonly used tunnels are pleural catheters. KPK is a minimal invasive surgical procedure that can be applied at the head of bed in the emergency service, in the intensive care unit, in the clinic.
