Amaç: Canlı vericili karaciğer transplantasyonlarında (CVKT) hepatik arter rekonstrüksiyonu esnasında tanı konulan arteryel çalma sendromu’nun tedavisinde yapılan gastroduodenal arter ligasyonunun, “duct-to-duct” safra anastomozlarında komplikasyonlara olan etkisinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Bu çalışma retrospektif bir kohort çalışmadır. Dokuz Eylül Üniversitesi Hastanesi Genel Cerrahi Karaciğer Nakli Biriminde Haziran 2000-2017 yıllarında CVKT yapılan 18 yaşından büyük veaz 6 ay sağ kalımı olan hastalar çalışmaya dahil edildi. Hastaların demografik verileri, greft ağırlığı, soğuk iskemi süreleri, Child ve MELD skorları, kullandıkları immünossüpresif ajanlar ve duct-to-duct safra anastomozu yapılan olgularda gastroduodenal arter ligasyonu sonrası postoperatif biliyer komplikasyonların ortaya çıkıp çıkmadığı incelendi. Bulgular: CVKT yapılan 240 hastanın hepatik arter rekonstrüksiyonu esnasında intraoperatif olarak 11’inde (%4,5) Doppler Ultrasonografi ile gastroduodenal arter çalma sendromu tanısı konuldu. Hastaların safra yolu anastomozları; 8(%72,7)’inde duct-to-duct anastomoz ve 3(%27,3)’ünde Roux-en-Y Hepatikojejunostomi şeklindeydi. Ortalama izlem süresi 3260 (371-4357) gündü. Takiplerinde duct-to-duct safra yolu anastomozu yapılan 8 olgunun hiç birisinde safra yolu komplikasyonu gelişmediği görüldü (p<0,001). Sonuç: CVKT’larında safra yolu devamlılığı “duct-to-duct” şeklinde olan hastalarda arteriyel çalma sendromu’nun tedavisinde gastroduodenal arter ligasyonu güvenle yapılabilir.
Purpose: The aim is to evaluate the effect of gastroduodenal arterial ligation in the treatment of arterial play syndrome diagnosed during hepatic artery reconstruction in living donor liver transplantations (CVKT) on complications in the "duct-to-duct" yellow anastomosis. Instrument and Method: This study is a retrospective cohort study. The 9th September University Hospital General Surgery Liver Transplant Unit in June 2000-2017 CVKT was conducted by patients older than 18 years and over 6 months of survival were included in the study. The demographic data of patients, the greft weight, the periods of cold scem, the Child and MELD scores, the immunosuppressive agents they use and the duct-to-duct gland anastomosis in the cases of gastroduodenal arterial ligation were studied whether postoperative arterial complications occurred. Results: In the course of the hepatic artery reconstruction of 240 patients with CVKT, intraoperatively in 11 (4,5%) Doppler Ultrasound was diagnosed with gastroduodenal artery play syndrome. Patients had anastomosis of the gland; anastomosis duct-to-duct in 8(72.7) and anastomosis Roux-en-Y in 3(27.3) were in the form of hepatikojejunostomy. The average monitoring time is 3260 (371-4357) days. In their follow-up, there was not any of the 8 phenomena of duct-to-duct gastrointestinal anastomosis that had developed a complication of the gastrointestinal path (p<0,001). The result: Gastroduodenal arterial ligation can be done safely in the treatment of arterial play syndrome in patients with a continuity of the gland path in the CVKTs in the form of "duct-to-duct".
