Amaç: Bu çalışmanın amacı, erken post-travmatik nöbeti önlenmede levetirasetamın ve fenitoinin etkinliğini karşılaştırmak ve klinik nöbet insidansını belirlemektir. Gereç ve Yöntem: Çalışmaya orta-şiddetli travmatik beyin hasarı tanısı alan, nörogörüntülemede beyin hasarı ile uyumlu bulgusu olan erişkin hastalar dahil edildi. Hastalar nöbet profilaksisi için uygulanan antiepileptik ilaca göre fenitoin grubu ve levetirasetam grubu olmak üzere iki gruba ayrıldı. Bulgular: Ocak 2012 ile Haziran 2017 arasında 100 travmatik beyin hasarlı hasta bu çalışmaya dahil edilme kriterlerini karşıladı. Nöbet profilaksisi, bu hastaların 60’ında fenitoin ve 40’ında levetirasetam ile sağlandı. Erken post-travmatik nöbet insidansı ise %8 idi. Gruplar arasında erken post-travmatik nöbet oranı açısından anlamlı farklılık saptanmadı (p>0,05). Travmatik beyin hasarı sonrası klinik nöbet insidansı %10 idi. Sonuç: Bu rapor, post-travmatik nöbet profilaksisinde levetirasetamın ve fenitoinin benzer etkinliğe sahip olduğunu gösterdi. Çalışmamız retrospektif bir tasarımda olmasına rağmen iki grup yaş, cinsiyet, travma mekanizması, nörogörüntüleme bulguları, Glasgow koma skoru, Marshall skoru ve akut fizyoloji ve kronik sağlık değerlendirmesi-II skorunu açısından benzer özelliklere sahipti. Bu bulgular, nöbet açısından fenitoin ve levetirasetamı karşılaştırılmayı güvenilir kılmaktadır.
Purpose: The purpose of this study is to compare the effectiveness of levetirasetam and phenitoin in the prevention of early post-traumatic attacks and to determine the incidence of clinical attacks. Tools and Methods: Study included adult patients with diagnosed mid-intensive traumatic brain damage, with findings compatible with brain damage in neurotransformation. Patients were divided into two groups according to the antiepileptic medicine used for the prevention of the stroke, the phenitoin group and the levetirasetam group. Results: Between January 2012 and June 2017, 100 traumatic brain injury patients met the criteria for inclusion in this study. The prevention of the tumor was provided with phenitoin in 60 of these patients and levetirasetam in 40. The early post-traumatic incidence was 8%. There was no significant difference in the rate of early post-traumatic attacks between the groups (p>0,05). The incidence of clinical attacks after traumatic brain injury was 10%. Result: This report showed that levetirasetam and phenitoin have a similar effect in the prevention of post-traumatic attacks. Although our study was in a retrospective design, the two groups had similar characteristics in terms of age, gender, trauma mechanism, neurodegeneration findings, Glasgow coma score, Marshall score and acute physiology and chronic health assessment-II score. These findings make the comparison of phenitoin and levetirasetam reliable in terms of infection.
