We report a case of aberrant left-sided gallbladder, in which the gallbladder is located on the left of the falciform and round ligaments, is a rare congenital anomaly. Anomaly was incidentally detected intraoperatively during laparoscopic cholecystectomy and preoperative work-up was normal. Laparoscopic cholecystectomy was safely performed and completed uneventfully. Although location of the cystic duct and artery were normal in our case, when incidentally recognized during surgery, surgeon must be aware of the unpredictable confluence of the cystic duct in to the common bile duct and vascular anomalies. Some operative precautions are useful for avoiding unwanted injuries to structures: The surgeon should keep in mind to rearrange the trocar sites and anterograd dissection of gallbladder if necessary, prepare and clip the cystic duct as close as possible to the infindibulum, perform an intraoperative cholangiography if in doubt.
