Amaç: Karbapenem dirençli Klebsiella pneumoniae’nin neden olduğu enfeksiyonların tedavisi ülkemizde ve dünyada çok ciddi sorundur. Karbapenem direnci bakteride çeşitli mekanizmalar ile oluşmakta ve bunun en sık nedeni karbapenemaz enzim üretimidir. Çalışmamızın amacı; hastanemizde 2012-2020 yılları arasında direnç oranlarındaki değişiminin ve dirence neden olan karbapenemaz enzim tiplerinin araştırılmasıdır. Yöntem: 2012-2020 yılları arasında laboratuvarımıza gönderilmiş çeşitli klinik örneklerden izole edilmiş K. pneumoniae suşlarından, karbapenemlere dirençli olarak bulunmuş olanlar çalışmaya dâhil edilmiştir. Bu suşlarda KPC, OXA-48, NDM, IMP ve VIM direnç genleri polimeraz zincir reaksiyonu ile araştırılmıştır. Bulgular: 2012-2020 yılları arasında toplam 291 (%7.5) adet karbapenem dirençli K. pneumoniae suşu bulunmuştur. Suşların sahip olduğu direnç genlerinin dağılımı; tek bir direnç genine sahip suşların sayısı OXA- 48, NDM-1, KPC ve VIM sırasıyla, 113 (%38.8), 32 (%11.0), 7 (%2.4), 5 (%1.7). Birden fazla direnç genine sahip suşların dağılımı ise şöyleydi: OXA-48+NDM 123 (%42.3), OXA-48+KPC iki adet, OXA-48+VIM iki adet, NDM+KPC bir adet ve NDM+IMP bir adet olarak bulunmuştur. Sonuç: Hastanemizde, karbapenem dirençli K. pneumoniae suşları yıllar içinde giderek artan oranda bulunmuştur. OXA-48 direnç geni en sık (%38.8) tekil gen olarak bulunmuştur. NDM direnç geni ise ikinci sıklıkta bulunmuştur. Bunun yanında ise OXA-48+NDM ikili gen varlığı hastanemiz en sık (%42.3) direnç mekanizması olarak bulunmuştur. Bu nedenlerle, KDKp suşlarında düzenli epidemiyolojik taramalarının ve direnç mekanizmalarının bölgesel ve ulusal olarak araştırmalarının yapılmasının bu bakteriye bağlı enfeksiyonların tedavisinde ve direncin kontrolünde çok önemli olduğu görülmektedir.
Purpose: Treatment of infections caused by Carbapenem-resistant Klebsiella pneumoniae is a very serious problem in our country and in the world. Carbapenem resistance is formed by various mechanisms in the bacteria and the most common reason for this is the production of carbapenemaz enzymes. The aim of our study is to investigate the changes in resistance rates in our hospital between 2012-2020 and the types of carbapenemase enzymes that cause resistance. Method: The K. pneumoniae juices, which were isolated from various clinical samples sent to our laboratory between 2012-2020, were found resistant to carbapenems, were included in the study. The KPC, OXA-48, NDM, IMP and VIM resistance genes have been studied with polymerase chain reaction. Results: A total of 291 (2012-2020 years) (7%). 5) a carbapenem resistant K. pneumoniae juice was found. The distribution of the resistance genes that the shells possess; the number of shells with a single resistance genes is OXA-48, NDM-1, KPC and VIM respectively, 113 (38.8 %), 32 (11.0 %), 7 (2.4 %), 5 (1.7 %). The distribution of the shells with multiple resistance genes was as follows: OXA-48+NDM 123 (42.3%), OXA-48+KPC two, OXA-48+VIM two, NDM+KPC one and NDM+IMP one. The result: In our hospital, carbapenem-resistant K. pneumoniae acids have been found increasing over the years. The OXA-48 resistance gene was found as the most frequent (38.8%) single gene. The NDM resistance gene is found in the second frequency. In addition, the presence of OXA-48+NDM binary genes has been found as our hospital’s most frequent (42.3%) resistance mechanism. For these reasons, the conduct of regular epidemiological scans and regional and national research of resistance mechanisms in the KDKp is very important in the treatment of these bacterial-related infections and in the control of resistance.
