AMAÇ: Bu çalışmada baş-boyun cilt kanseri (BBCK) nedeniyle parotidektomi işlemi uygulanan hastaların tanı, tedavi ve takip süreçleriyle ilgili klinik deneyimimiz paylaşıldı. YÖNTEMLER: Ocak 2012 - Ocak 2020 tarihleri arasında BBCK nedeniyle elektif ve terapötik parotidektomi uygulanan 30 hasta (20 erkek 10 kadın; ort. yaş: 76±9.8 yıl; dağılım, 50-90 yıl) bu retrospektif çalışmaya dâhil edildi. BULGULAR: Elektif parotidektomi 11 (%36.7) hastada uygulandı ve terapötik parotidektomi 19 (%63.3) hastada yapıldı. Primer tümör histopatolojisi 23 hastada skuamöz hücreli karsinom, beş hastada bazal hücreli karsinom ve iki hastada malign melanom idi. Primer tümörün en sık görüldüğü lokalizasyonlar sırasıyla aurikula (n=9), şakak bölgesi (n=7) ve preauriküler bölge (n=6) idi. Hastaların 25’ine (%83.3) boyun diseksiyonu yapıldı. Hastaların 12’sine (%40) ameliyat sonrası radyoterapi uygulandı. Hastaların ortalama takip süresi 40.3±30.9 (dağılım, 6-99) ay olarak saptandı. Elektif parotidektomi yapılan hastalarda üç yıllık sağkalım %90 (9/10) ve beş yıllık sağkalım %77.8 (7/9) iken, terapötik parotidektomi yapılan hastalarda üç yıllık sağkalım %46.2 (6/13) ve beş yıllık sağkalım %22.2 (2/9) idi. Elektif parotidektomi hastalarının hem üç yıllık hem de beş yıllık sağkalım oranları terapötik parotidektomi hastalarından daha yüksek idi (p<0.05). SONUÇ: Baş-boyun cilt kanserli hastalarda başta parotis bezinde olmak üzere bölgesel metastaz ortaya çıkması durumunda sağkalım ileri derecede düşmektedir. Bu nedenle yüksek riskli hastalarda elektif parotidektomi uygulanması gerektiğini düşünmekteyiz. Terapötik parotidektomi gerektiğinde boyun diseksiyonu yapılmalıdır ve ameliyat sonrası patoloji raporuna göre radyoterapi gerekebileceği akılda tutulmalıdır. Goal: In this study, we shared our clinical experience of the diagnosis, treatment and tracking processes of patients who undergo the parotidectomy procedure due to head and neck skin cancer (BBCK). Methods: 30 patients with elective and therapeutic parotidectomy due to BBCK between January 2012 and January 2020 (20 men and 10 women; average. Age: 76±9.8 years; distribution, 50-90 years) was included in this retrospective study. Elective parotidectomy 11 (36.7%) was administered in patients and therapeutic parotidectomy 19 (63.3%) was administered in patients. Primary tumor histopathology was squamous cell carcinoma in 23 patients, basal cell carcinoma in five patients and malignant melanoma in two patients. The most common localizations of the primary tumor were, respectively, the auricula (n=9), the shake area (n=7) and the preauricular area (n=6). 25 percent of patients (83. 3) a disconnection was made. 12 patients (40%) were subjected to postoperative radiation therapy. The patient’s average tracking time was 40.3±30.9 (distribution, 6-99) months. Three-year survival was 90 percent (9/10) and five-year survival was 77.8 percent (7/9) while three-year survival was 46.2 percent (6/13) and five-year survival was 22.2 percent (2/9) in patients with therapeutic parotidectomy. The survival rates of elective parotidectomy patients were both three and five years higher than therapeutic parotidectomy patients (p<0.05). The result: in patients with head-to-course skin cancer, the survival is gradually reduced if regional metastasis occurs, primarily in the parotic gland. Therefore, we consider that elective parotidectomy should be applied to high-risk patients. When therapeutic parotidectomy is needed, the neck disection should be done and should be remembered that radiotherapy may be required according to the postoperative pathology report.
