Alt ekstremite periferik arter hastalığında endovasküler tedavi: Tek merkez deneyimi

GİRİŞ ve AMAÇ: Semptomatik alt ekstremite arter hastalığında endovasküler tedavi giderek artan önemle tedavide yerini almıştır. Çalışmamızda merkezimizin son 8 yılına ait perkütan işlem verilerinin retrospektif sunulması amaçlanmıştır. YÖNTEM ve GEREÇLER: Temmuz 2011 ile Aralık 2018 arası kliniğimizde yapılan alt ekstremite arterlerine yönelik girişimler geriye dönük tarandı. Demografik, laboratuvar ve işlem verileri hastane içi istenmeyen olaylar hasta kayıtlarından elde edilerek sunuldu. BULGULAR: 541 periferik arter girişimi tarandı. Toplamda 374 adet alt ekstremite arterine yönelik girişimde iliak (n=219), femoropliteal (n=184) ve tibial/peroneal (n=17) arterleri içeren 420 damara endovasküler tedavi çalışmaya dahil edildi. Genel işlem başarısı %97.1 olup işlem sayı ve başarısında yıllara göre belirgin artış izlendi. İliak ve femoropopliteal girişimlerde işlem başarısı %97’ nin üzerinde iken tibioperoneal arter girişimlerinde %100 saptandı. Hastane içi istenmeyen ölüm 3 (%0.8), amputasyon 2 (%0.5), major kanama 2 (%0.5), diyaliz ihtiyacı 3 (%0.8) ve serebrovasküler olay (%0.3) saptandı. Hastaların %85’ i işlem sonrası bir gün içinde taburcu edildi. TARTIŞMA ve SONUÇ: Alt ekstremite arter hastalıklarında endovasküler tedavi yıllar içinde giderek artan sayılarda yapılmaktadır. Teknik gelişmeler ve artan tecrübe yüksek başarı, makul komplikasyon oranları ve erken mobilizasyona imkan sağlamaktadır.

Endovascular Treatment of Peripheral Artery Disease in the Lower Extreme: Single Center Experience

INTRODUCTION AND OBJECTIVE: In the symptomatic lower end artery disease, endovascular treatment has been replaced with increasing importance in treatment. Our work is aimed at presenting retrospective data of our center’s percutaneous processing for the last 8 years. Method and Requirements: In the course of July 2011 to December 2018, the initiatives for the lower end arteries in our clinic were backward scaned. Demographic, laboratory and processing data were provided by obtaining from the patient records of unwanted events in the hospital. 541 peripheral artery attempts were scaned. In a total of 374 sub-extreme arteries, 420 vessels containing arteries (n=219), femoropliteal (n=184) and tibial/peroneal (n=17) were included in the endovascular treatment study. The overall success of the process is 97. 1 and the number of transactions and successes were observed significantly increased over the years. The success of the processing in gland and femoroplithic initiatives was above 97% while in tibioperoneal artery initiatives 100% was detected. In the hospital, unwanted deaths were detected 3 (%0.8), amputation 2 (%0.5), major bleeding 2 (%0.5), dialytic need 3 (%0.8) and cerebrovascular event (%0.3). 85% of patients were removed within a day after the treatment. INTRODUCTION and RESULTS: Endovascular treatment in lower end arterial diseases is carried out in increasing numbers over the years. Technical advances and increasing experience enable high success, reasonable complications rates and early mobilization.
