This clinical report describes the apexification procedure of a maxillary right central incisor that had a complicated crown fracture in a 7 –year- old girl. She fell down when running in the school garden and crashed her head into sidewalk. Her maxillary right incisor tooth was broken, she came to our clinic one day after the accident. We performed apexification procedure because of large size of the perforation area and bleeding from the pulp. The treatment procedure; firstly, the root canal was biomechanically prepared and medicated with calcium hydroxide [Ca(OH)2] to stimulate hard tissue barrier formation between visits. Then, permanent root canal filling was performed and tooth restorated
