Çocuklarda Mezenter Kistlerinin Cerrahi Tedavi Sonuçları: Tek merkez deneyimi

Abstract:

Amaç:

Mezenter kistleri (MK) nadir görülen intraabdominal selim tümörlerdir. Lenfatik, mezenkimal ve enterik doku kaynaklı olabilirler. Bunlara ek olarak ürogenital kistler, dermoid kist ve psödokistler de bu sınıf içinde incelenir. Her yaşta görülebilirler. Bu çalışmada MK konusunda cerrahi deneyimin paylaşılması amaçlanmıştır.

Yöntem:

2005-2017 yılları arasında çeşitli bulgularla kliniğimize başvuran ve ameliyatlarında MK saptanan hastalar geriye dönük olarak tarandı.

Bulgular:

Yaş ortalaması 38,73 ay (15 gün-10 yıl) olan 17 MK’li hasta mevcuttu. Hastaların 9’u (%53) erkek ve 8’i (%47) kızdı. Yedi hasta karın ağrısı, 7 hasta karın şişliği, 6 hasta kusma ve/veya 1 hasta alt ekstremitede ödem nedeniyle başvurdu. Hastaların 2’si antenatal tanılıydı. Tüm hastalar ultrasonografi (US) ile değerlendirildi. Ayrıca 9 hastaya bilgisayarlı tomografi (BT) ve/veya manyetik rezonans görüntüleme (MRG) yapıldı. Cerrahi girişim olarak 14 hastada total kist eksizyonu uygulandı. Ek olarak bu hastaların 1’ine parsiyel gastrektomi,1’ine ileo-kolonik rezeksiyon anastomoz, 2’sine jejunal rezeksiyon anastomoz ve 4’üne ise, ileal rezeksiyon anastomoz yapıldı. Kalan 3 hastaya kist aspirasyonu ve fenestrasyon uygulandı. Çıkarılan kistlerin histopatolojik incelemesi, lenfanjiom (8), mezenkimal kist (4), enterik kist (2), dermoid kist (1) ve psödokist (1) olarak sonuçlandı. Bir hastada histopatolojik örnek alınmamıştı. Takipte bir hastada rezidü kistik yapılar nedeniyle ileri tedavi gerekti.

Sonuç:

MK doğumsal bir anomalidir. Bununla birlikte, herhangi bir yaşta semptom verebilir. Histopatolojik olarak farklı alt gruplarda değerlendirilse de tedavisi tam cerrahi rezeksiyondur. Tutulum yerine ve derecesine göre, cerrahi rezeksiyonun kapsadığı alan değişebilir. Tam olarak rezeksiyon yapılabilen hastalarda uzun dönem takip sonuçları son derece iyidir.

The Surgery Results of Mezenter Cysts in Children: One-Center Experience

The purpose:

Mezenter cysts (MK) are rare intraabdominal selim tumors. They can be caused by lymphatic, mesenchimal and enteric tissues. In addition, urogenital cysts, dermoid cysts and psedocysts are also studied within this class. They can be seen at any age. This study is aimed at sharing surgical experience with MK.

The Method:

Between 2005 and 2017, the patients who appealed to our clinic with various findings and diagnosed MK in their operations were backward scaned.

The findings:

The average age was 38.73 months (15 days to 10 years) and there were 17 MK patients. 9 of the patients (53 percent) were male and 8 (47 percent) were angry. Seven patients suffered from abdominal pain, seven patients suffered from abdominal swelling, six patients suffered from vomiting and/or one patient suffered from swelling in the lower limbs. Two of the patients were identified with ananthate. All patients were evaluated by ultrasound (US). In addition, 9 patients received computer tomography (BT) and/or magnetic resonance imaging (MRG). A total of 14 patients were diagnosed with cyst excision as a surgical initiative. In addition, 1 of these patients received partial gastrectomy, 1 was anastomosis of the eco-colonial resection, 2 was anastomosis of the jeunal resection and 4 was anastomosis of the eco-resection. The remaining 3 patients were applied cyst aspiration and fenestration. The histopathological examination of the extracted cysts resulted in lymphangioma (8), mesenchimal cyst (4), enteric cyst (2), dermoid cyst (1) and psedocyst (1). There was no histopathological sample in a patient. A patient in follow-up needed advanced treatment due to residuous cystic structures.

The result:

MK is an abnormality. However, it can give symptoms at any age. Although histopathologically evaluated in different subgroups, the treatment is a complete surgical resection. Depending on the place and degree of holding, the area covered by the surgical resection may vary. The long-term tracking results are extremely good in patients who can perform exact resection.
