Metabolik sendromlu hastalarda serum irisin düzeyi ile kardiyovasküler risk faktörleri ve insülin direnci parametreleri arasındaki ilişki

GİRİŞ ve AMAÇ: Çalışmamızın amacı, metabolik sendromlu (MetS) hastalarda serum irisin düzeyi ile insülin direnci belirteçleri ve kardiyovasküler hastalık (KVH) risk faktörleri arasındaki ilişkiyi saptamaktır. YÖNTEM ve GEREÇLER: Uluslararası Diyabet Federsayonuna (IDF) göre MetS’li 46 hasta ve 26 kontrol grup hastası dahil edildi. Serum irisin düzeyi, KVH risk faktörleri (cinsiyet, yaş, obezite, lipid bozuklukları, sigara, hipertansiyon (HT), aile öyküsü) ve insülin direnci belirteçleri (HOMA-IR, Trigliserid (Trg)/Yüksek yoğunluklu lipoprotein (HDL), bel çevresi (BÇ) ve vücut kitle indeksi (VKİ) çalışıldı. BULGULAR: Çalışmaya dahil edilen hastaların 28’i erkek (%42), 38’i (%58) kadın idi. Ortalama yaş 43.78±14.16 idi. 25 (%38) hastada HT, 19’ unda (%29) obezite, 31’ inde (%47) sigara içiciliği, ve 32’ sinde ( %48) ailede KVH öyküsü mevcuttu. Tüm çalışma hastalarının ortalama serum irisin düzeyi 7.27 ± 4.65 idi. Serum irisin düzeyi kontrol grubu ile karşılaştırıldığında metabolik sendromlu hastalarda anlamlı olarak daha düşük idi (5.47 ± 2.51 vs 10.06 ± 5.47, p <0.001). Yaş ile serum irisin düzeyi arasında negatif korelasyon saptandı. Serum irisin düzeyi ile cinsiyet, sigara içiciliği, ailede KVH öyküsü, HDL, LDL, Trg, total kolesterol düzeyleri arasında istatistiksel anlamlı ilişki saptanmadı (p,0.502; p,0.974; p,0.264; p,0.783; p,0.648; p,0.286; p,0.128, respectively). Serum irisin düzeyi, HbA1c hariç, insülin direnci belirteçleri (HOMA-IR, Trg / HDL, WC/HC, and BMI) ile korele saptanmadı. Serum irisin düzeyi ile HbA1c arasında negatif korelasyon saptandı (B=-0.935, p=0.03). TARTIŞMA ve SONUÇ: Serum irisin düzeyi MetS’ li hastalarda anlamlı olarak daha düşük saptandı. HbA1c ve yaş hariç, KVH risk faktörleri ve insülin direnci belirteçleri arasında anlamlı ilişki saptanmadı.

The aim of our study is to identify the relationship between the level of serum iris in patients with metabolic syndrome (MetS) and insulin resistance indicators and cardiovascular disease (KVH) risk factors. Method and Requirements: According to the International Diabetes Federation (IDF), 46 patients with MetS and 26 patients in the control group were included. Serum iris levels, KVH risk factors (gender, age, obesity, lipid disorders, smoking, hypertension (HT), family history) and insulin resistance indicators (HOMA-IR, Triglyceride (Trg)/High-intensity lipoprotein (HDL), back circle (BC) and body mass index (VKI) were studied. 28 of the patients involved in the study were male (42%) and 38 (58%) female. The average age was 43.78 ± 14.16. 25 (38%) patients had HT, 19 (29) obesity, 31 (47) smoking, and 32 (48) families had KVH history. The average level of serum iris in all study patients was 7.27 ± 4.65. Serum irisine levels were significantly lower in patients with metabolic syndrome compared to the control group (5.47 ± 2.51 vs. 10.06 ± 5.47, p <0.001). Negative correlation between age and serum iris levels was identified. There is no statistically meaningful relationship between the levels of serum iris and gender, smoking, family KVH history, HDL, LDL, Trg, total cholesterol levels (p,0.502; p,0.974; p,0.264; p,0.783; p,0.648; p,0.286; p,0.128, respectively). Serum irisine levels, except HbA1c, are not correlated with insulin resistance indicators (HOMA-IR, Trg / HDL, WC/HC, and BMI). A negative correlation between the serum irisine level and HbA1c was found (B=-0.935, p=0.03). CONTRACT and RESULT: Serum iris levels were significantly lower in MetS patients. Except for HbA1c and age, there is no meaningful relationship between KVH risk factors and insulin resistance indicators.
