PYOJENİK GRANÜLOMDAN GELİŞEN PERİFERAL OSSİFİYE FİBROMA: OLGU SUNUMU

Abstract:

Periferal ossifiye fibroma, genç ve erişkin yaştaki kadın hastalarda görülen, sıklıkla anterior alanlarda meydana gelen reaktif diş eti büyümesidir. Periferal ossifiye fibroma ağrısız seyreder ve semptom verene kadar hekimler tarafından gözden kaçırılmaktadır. Periodontal ligamentten kaynaklandığı düşünülmektedir ve %9 oranında diş etinin aşırı reaksiyonu şeklinde oluşur. Zayıf yapılan restorasyonlar ve lokal irritasyonlar sebepleri arasında gösterilmektedir. Bu lokal irritasyonlar arasında travma, dental plak, diş taşı, mikroorganizmalar, diş protezi vardır. Klinik ve histopatolojik olarak birçok benzerliğinin bulunması ve fibröz maturasyon ve daha sonra meydana gelen kalsifikasyonlar sebebiyle bazı periferal ossifiye fibroma’ların ilk olarak piyojenik granülomadan geliştiğine inanılmıştır. Bu olgu sunumunda, 38 yaşındaki kadın hastada, ağız içinde sağ mandibular premolar bölgede,  12 haftalık gebe iken farkedilen fakat daha sonra hastanın ihmal etmesi üzerine dokunun fibroz doku yönünde olgunlaşması ile sert bir kıvam kazanan şişliğin kilinik ve histopatolojik inceleme neticesinde periferal ossifiye fibroma tanısının konması ve cerrahi tedavisi sunuldu. Peripheral ossifying fibromas are painless and are missed by physicians until they give symptoms.It is thought to originate from the periodontal ligament and occurs as an overreaction of 9% of the gum. Poor restorations and local irritations are shown as reasons. These local irritations include trauma, dental plaque, dental calculus, microorganisms, dental prosthesis. It was believed that some peripheral ossifying fibroma developed from pyogenic granulomas due to the presence of many similarities clinically and histopathologically and with fibrous maturation and subsequent calcifications. In this case report, a 38-year-old female patient was noticed in the right mandibular premolar region in the mouth, while the lesion was 12 weeks pregnant.however, the lesion had a hard consistency with the maturation of the fibrous tissue upon neglect of the patient.The diagnosis and surgical treatment of peripheral ossified fibroma are presented as a result of clinical and histopathological examination of the swelling. Abstract:

Periferal oscillation fibrom is a reactive tooth growth that occurs in young and adult female patients, often occurring in the frontal areas. The fibroma of the peripheral oscillation looks painless and is missed by doctors until the symptoms are given. It is believed to be caused by the periodontal ligament and is formed as an excessive reaction of the tooth at 9%. Weak restorations and local irritations are shown among the causes. These local irritations include trauma, dental plaque, tooth stones, microorganisms, tooth protection. There are many clinical and histopathological similarities, and it is believed that some peripheral oscillated fibromes first developed from pyogenic granuloma due to fibrosis maturity and subsequent calcifications. In the presentation of this fact, a 38-year-old woman in the patient, the right mandula in the mouth in the premolar area, was noticed during 12 weeks of pregnancy but later the patient ignored the tissue fibrosis tissue mature with a severe decrease of the swelling and histopathological examination resulted in the placement of the diagnosis of the peripheral ossiphic fibrom and the surgical treatment. Peripheral ossified fibromas are usually seen in young and adult female patients, often in the previous areas of the reactive gingival enlargement. Peripheral ossifying fibromas are painless and are missed by physicians until they give symptoms.It is thought to originate from the periodontal ligament and occurs as a overreaction of 9% of the gum.
