BENİGN ENDİKASYONLARLA YAPILAN HİSTEREKTOMİLERDE PREOPERATİF SERVİKAL SİTOLOJİK İNCELEME YAPILMASI ZORUNLU MUDUR?

Abstract:

Amaç: Bu çalışmada amacımız benign nedenlerle histerektomi yapılan olgularda preoperatif servikal sitolojik inceleme yapılan ve yapılmayanlarda postoperatif servikal histopatolojik bulguların karşılaştırılmasıdır. Gereç ve yöntem: Bu kesitsel çalışmada benign jinekolojik endikasyon ile histerektomi yapılan 215 olgu, histerektomi öncesinde servikal sitolojik inceleme yapılıp sonucunda anormal sitolojik bulgu izlenmeyenler (Grup I, n=151) ile servikal sitolojik inceleme yapılmamış olanlar veya smear sonucu “yetersiz materyal” nedeniyle tanısal olmayıp sitoloji tekrarı yapılmamış olanlar (Grup II, n= 64) olarak ikiye ayrıldı. Histerektomi öncesinde bilinen servikal intraepitelayal lezyon veya anormal sitolojik bulgusu olan olgular ile jinekolojik malignite nedeniyle histerektomi yapılan hastalar çalışma dışında bırakıldı. İki grubun postoperatif servikal histopatolojik sonuçları incelendi ve anormal sonuç saptanma oranları karşılaştırıldı. İki grup arasındaki karşılaştırmalar Bağımsız örnek T-Test, Mann-Whitney testi ve Ki-kare testi kullanılarak yapıldı. p<0.05 değeri istatistiksel olarak anlamlı kabul edildi. Bulgular: Serviksin postoperatif histopatolojik inceleme sonucunda her iki grupta da preinvaziv lezyon saptanma oranı benzer bulundu (%7.3’e karşı %10.9, p= 0.377). Grup 1’de 7 olguda düşük dereceli servikal intraepitalyal lezyon (LSIL/CIN 1) 3 olguda ise yüksek dereceli servikal intraepitalyal lezyon (HSIL /CIN 2-3), 1 olguda ise adenokarsinoma in situ saptandı. Grup II’de ise 4 olguda düşük dereceli servikal intraepitelyal lezyon (LSIL/CIN 1) 3 olguda ise yüksek dereceli servikal intraepitalyal lezyon (HSIL/CIN 2-3) saptandı. Her iki grupta da invaziv servikal malignite izlenmedi. Sonuç: Benign endikasyonla yapılacak histerektomi öncesinde rutin servikal kanser tarama programı dışında servikal sitolojik inceleme yapılmadan operasyonun uygulanması insidental servikal preinvaziv/invaziv lezyon ile karşılaşma riskini artırmamaktadır

Is it necessary to perform a preliminary service cytological examination in hysterectomies performed with benign indications?

Purpose: In this study, our aim is to compare postoperative cervical histopathological findings in cases of preoperative cervical cytological examination in cases of hysterectomy for benign reasons and not in cases of postoperative cervical histopathological findings. In this cutting study, the 215 facts of hysterectomy with benign gynecological indications, the cervical cytological examination before hysterectomy and the result of an abnormal cytological findings (Group I, n=151) and the cervical cytological examination not performed or the result of the smar was not diagnosed due to "inadequate material" and the cytological repeat (Group II, n=64) were divided into two facts. Patients with known cervical intraepithelial injury or an abnormal cytological manifestation before hysterectomy and hysterectomy due to gynecological malignity were left out of study. The postoperative cervical histopathological results of the two groups were studied and the rate of detection of an abnormal result was compared. Comparisons between the two groups were made using Independent sample T-Test, Mann-Whitney test and Ki-Care test. The value p<0.05 was statistically considered meaningful. Results: The postoperative histopathological examination of the cervical body found the same rate of detection of pre-invasive injury in both groups (7.3% vs. 10.9% p= 0.377). In group 1 in 7 cases low-degree cervical intraepital injury (LSIL/CIN 1) in 3 cases high-degree cervical intraepital injury (HSIL/CIN 2-3), in 1 cases adenocarcinoma in situ was detected. In Group II, there were 4 low-degree cervical intraepithelial injury (LSIL/CIN 1) and 3 high-degree cervical intraepithelial injury (HSIL/CIN 2-3). In both groups, invasive cervical malignity was not observed. Result: Except for routine cervical cancer screening program before hysterectomy with benign indication, the application of the surgery without cervical cytological examination does not increase the risk of incidental cervical pre-invasive/invasive injury
