Amaç: Bu çalışmanın amacı, yenidoğan yoğun bakım ünitesine yatırılan geç (34-36 gebelik haftası) ve orta derece (32-33 gebelik haftası) prematüre olguların perinatal ve klinik problemlerini retrospektif olarak değerlendirmektir. Gereç ve Yöntem: Ekim 2016 ve Ekim 2018 arasında Derince Eğitim ve Araştırma / Alikahya Hastanesi yenidoğan yoğun bakım ünitesine yatan 208 geç ve orta prematüre bebekler bu çalışmaya dahil edildi. Prematüre bebeklerin anne yaşı, paritesi, maternal risk faktörleri, gebelik haftası, doğum şekli, doğum ağırlığı, neonatal dönem sorunları ve yatış nedenleri, mekanik ventilasyon gereksinimleri, hastanede yatış süreleri incelendi. Bulgular: 208 hastadan 105 (%50.4) geç prematüre ve 103 (%49.6)'ü orta derece preterm idi. Yatış endikasyonları incelendiğinde, 176'sınında (%84) solunum sıkıntısı, 12'inde (%5.7) yenidoğan enfeksiyonu, 4'ünde (%1.9) hipoglisemi, geriye kalanların ise konjenital anomali, hiperbilirubinemi ve asfiksi tanısı ile izlendiği bulundu. Orta derece prematürlerde, geç prematür bebeklere göre respiraruar distress sendromu (RDS), sürfaktan ihtiyacı, nazal sürekli hava yolu basıncı (CPAP) süresi ve suplemental oksijen süresi, tam enteral beslenmeye geçiş zamanı, doğum kilosuna ulaşma zamanı ve taburcu edilme zamanlarında, istatistiksel olarak anlamlı ölçüde artış saptandı. Sonuç: Gebelik haftası küçüldükçe prematürelikten kaynaklanan klinik sorunların oranı da artmaktadır. Prematüre bebeklerde yatış gerektiren en önemli morbiditeler arasında solunum sıkıntısı gelmektedir.
Purpose: The aim of this study is to retrospective assess the perinatal and clinical problems of late (34-36 weeks of pregnancy) and middle (32-33 weeks of pregnancy) premature occurrences placed in the newborn intensive care unit. Tools and Methods: 208 late and middle premature babies, based on the newborn intensive care unit in Deep Education and Research / Alikahya Hospital between October 2016 and October 2018, were included in this study. Early-born babies were examined by the mother's age, parity, maternal risk factors, pregnancy week, form of birth, birth weight, neonatal period problems and causes of infertility, mechanical ventilation requirements, hospitalization periods. Results: from 208 patients, 105 (50.4%) were late premature and 103 (49.6%) were middle-degree premature. When sleeping indications were studied, 176 (84 percent) were diagnosed with respiratory failure, 12 (5.7 percent) were infected with newborn infection, 4 (1.9 percent) were hypoglycemic and the rest were diagnosed with congenital anomaly, hyperbilirubinemia and asphyxia. In mid-degree premature, late premature babies, the respiratory distress syndrome (RDS), the need for drilling, the period of nasal constant air pressure (CPAP) and the supplemental oxygen time, the time of transition to full intravenous nutrition, the time of achieving the birth weight and the time of discharge, statistically significantly increased. The result is that as the week of pregnancy decreases, the rate of clinical problems caused by prematurity also increases. In premature babies, the most important morbidities that require sleep are breathing disorders.
