Post-operatif (post-op) dönemde solunum sistemine ilişkin sorunlar mortalite ve morbiditenin artmasında en önemli nedenler arasındadır. Endotrakeal entübasyon sonrası görülen trakea patolojilerinden biri trakeal stenozdur. Yüksek endotrakeal tüp kaf basıncı bu komplikasyonun gelişmesinde önemli rol oynar. Stenozlarda cerrahi kararı, solunum fonksiyon bozukluğunun derecesine göre verilir. Mitral kapak replasmanı ve triküspit annüloplastisi yapılan 67 yaşındaki kadın hasta post-op yoğun bakım ünitemize entübe olarak kabul edildi. Hastanemizde yattığı bir aylık süre içinde toplam 4 kere endotrakeal entübasyon öyküsü olan hastada geçmeyen solunum sıkıntısı nedeniyle etiyoloji araştırıldığında laringeal granüloma ve subglottik darlık olduğu görüldü. Bu yazıda açık kalp cerrahisi geçiren hastada post-op dönemde görülen solunum sıkıntısında ayırıcı tanının nasıl yapıldığı bir olgu üzerinden anlatılmaktadır.
Problems with the respiratory system in the post-operative (post-op) period are among the main causes of increased mortality and morbidity. One of the pathologies of trachea observed after endotrakeal entubation is tracheal stenosis. High endotrakeal tube head pressure plays an important role in the development of this complication. The surgical decision in stenosis is given according to the degree of respiratory disorder. A 67-year-old woman with mitral cover replacement and trichyspithic anuloplasty was considered an enthube to our post-op intensive care unit. The patient, who had a total of 4 times endotrakeal entubation history during a month in our hospital, was studied at the etiology due to respiratory failure, with laringeal granuloma and subglothic narrowness. This article describes how a distinctive diagnosis of respiratory failure in the post-op period is made in a patient who is undergoing open heart surgery.
