Batı Karadeniz Bölgesi Karabük İli ve Çevresinde Temporomandibular Eklem Disfonksiyon Prevalansının Değerlendirilmesi

Abstract:

Amaç: Bu araştırmanın amacı Karabük ili ve çevresinden temporomandibular eklem (TME) disfonksiyonu şikâyeti ile kliniğimize başvuran hastaları yaş, cinsiyet, semptom ve disfonksiyonun türü açısından değerlendirmektir.  Yöntemler: Çalışmaya, Aralık 2016-Mart 2018 tarihleri arasında, kliniğimize TME disfonskiyonu ile başvuran 233 hasta dahil edilmiştir. İlgili muayeneler tek bir hekim tarafından, bilateral olarak yapılmıştır. TME bozukluğu teşhisi konulurken, Temporomandibular Rahatsızlıklar/ Teşhis Kriterleri Eksen I (TMR/TK Eksen I)’den ve manyetik rezonans görüntülemeden yararlanılmıştır.  Bulgular: Çalışmamızın sonuçlarına göre, 19-48 yaş aralığındaki bireylerde ve kadınlarda TME disfonksiyon görülme prevalansı istatistiksel olarak anlamlı derecede yüksek bulunmuştur (p<0.001, p<0.05). En belirgin semptom ağrıydı ve bunu sırasıyla eklemde klik sesi ve krepitasyon takip etti. Teşhis edilen TME disfonksiyonlarının türü-hasta sayısı yönünden değerlendirmede ise Redüksiyonlu Disk Deplasmanı tanısına sahip hasta sayısı diğer disfonksiyon türlerine sahip hasta sayılarına göre istatistiksel olarak anlamlı düzeyde daha fazla bulunmuştur (p<0.001).  Sonuç: Bizim bölgemizdeki TME disfonksiyonlarının büyük çoğunluğu genç ve yetişkinlerde ve daha çok kadınlarda ortaya çıkan Redüksiyonlu Disk Deplasmanı (kas problemlerinin eşlik etmediği) olarak gözükmektedir.

Purpose: The purpose of this research is to evaluate patients who apply to our clinic with a temporomandibular joint dysfunction (TME) complaint from Karabük and its surroundings in terms of age, gender, symptoms and type of dysfunction.  Methods: In the study, between December 2016-March 2018, 233 patients were included in our clinic with TME dysphonskiyon. Interested examinations are carried out by a single doctor, bilateral. During the diagnosis of TME disorder, Temporomandibular disorders/Diagnostic Criteria Axis I (TMR/TK Axis I) and magnetic resonance imaging were used.  Results: According to the results of our study, the prevalence of TME dysfunction in individuals and women aged 19-48 was statistically significantly high (p<0.001, p<0.05). The most obvious symptom was pain and it followed by the click voice and crepitation in the attachment accordingly. In the assessment of the type-number of diagnosed TME disorders, the number of patients diagnosed with Reduced Disk Movement was more statistically significant than the number of patients with other types of disorders (p<0.001).  The result: The vast majority of TME disorders in our region appear to be Reduced Disk Movement (not accompanied by muscle problems) that occurs in young people and adults and more in women.

Aim: The purpose of this study is to evaluate the patients who complain of temporomandibular joint (TMJ) dysfunction from Karabük province and its around city in terms of age, sex, symptom, and type of dysfunction.  Methods: The study included 233 patients who applied with TMJ dysfunction in our clinic between December 2016 and March 2018. The related examinations were performed bilaterally by a single doctor. Diagnosis of TMJ dysfunction was made by taking Diagnostic Criteria /Temporomandibular Disorders Axis 1 criteria and magnetic resonance imaging into consideration.  Results: According to the results of our study, the prevalence of TMJ dysfunction in individuals aged 19-48 years and in women was found to be statistically signifi-cantly higher (p<0.001, p<0.05). The primary symptom was pain and the following symptoms were sounds of clicking and crepitations in all patients, respectively. When assessing the type of TMJ dysfunction diagnosed, the number of patients with Disc Displacement with reduction was found to be statistically significantly higher than the patients with other types of dysfunction (p<0.001).  Conclusion: Our results show that TMJ dysfunction is mostly appeared in young and adult patients and female who have Disc Displacement with Reduction (without chewing muscular problems).
