KRONİK KARIN AĞRISININ NADİR GÖRÜLEN BİR NEDENİ: APENDİKS MUKOSELİ. OLGU SUNUMU

Amaç: Bu çalışmadaki amaç kronik karın ağrısının nadir görülen bir nedeni olan ve  apendiks lümeninin mukus ile distansiyonu sonucu gelişen apendiks mukoselinde tanı yöntemlerinin etkinliğini ve cerrahi tedavi sonrası sonuçları sunmaktır.Vaka Sunumu: Kırk yedi yaşında bayan hasta altı aydır olan karın ağrısı şikayeti ile acil servise başvurdu. Fizik muayenede karın sağ alt kadranda hassasiyet ve aynı lokalizasyonda ele gelen kitle lezyon mevcuttu. Tüm batın ultrasonografisi ve tüm batın tomografisinde apendiks mukoseli olabilecek kitle lezyon saptandı.    Sonuç: Hasta apendiks mukoseli ön tanısı ile ameliyata alındı. Eksplorasyonda 10 santimetre uzunluğunda ve 1,5 santimetre çapında radiks kısmı çekum lümenine doğru geniş tabanlı olarak uzanan apendiks görüldü.  Hastaya apendektomi ve çekum wedge rezeksiyonu yapıldı. Hasta ameliyat sonrası 4. gün sorunsuz olarak taburcu edildi. Ameliyat sonrası 6. ayında olan hastanın herhangi bir şikayeti bulunmamaktadır.

Abstract:

Purpose: The purpose of this study is to provide the effectiveness of the diagnosis methods in the apendix mucus, which is a rare cause of chronic abdominal pain and develops as a result of the distance of the apendix lumen with the mucus, and the results of post-chirurgical treatment.Case Presentation: The fourteen-seven-year-old woman, who was a patient, applied to the emergency service with a complaint of abdominal pain for six months. In the physical examination there was sensitivity in the lower right body of the abdomen and mass injury addressed in the same location. In the entire western ultrasound and in the entire western tomography, mass injury that may be appendic mucus was detected.    The result: the patient was taken to surgery with a pre-diagnosis of the apendicus mucus. In the exploration, an apendix, which was 10 cm long and 1.5 cm in diameter, was found on a wide base toward the drawing lumen of the radical part. The patient was given apendectomy and drag wedge resection. The patient was dismissed on the 4th day after the surgery. The patient in the sixth month after the surgery has no complaints.
