Abstract The population aging brings the increase of the dependence situations associated to the age. In countries where the sanitary resources are accessible to most of the population, the number of older people increases, as it happens in the cases of Spain and Cuba. In this work, it interested to know, settling down comparative, the advances and challenges in relation to the attention to the dependence and cares among these countries. We used a qualitative methodology and of a no systematic revision of scientific articles and official reports of Cuba and Spain. We obtained results related with the mixed pattern of attention of cares in Spain in which combine the formal care with the informal one. The growing population rate in dependence situation, associate or not to the age that takes place as much in Cuba as in Spain that outlines a challenge for the respective states. In the case of Cuba inequalities to the interior of the process of care and the necessity of seeing it as a central axis in the systems of social protection. Among the main summations they stand out that is important to get the effectiveness of the system, but that their attainment can´t base on any case in the decrease of the quality or the quantity of the resources. For it, it is considered that the States should enlarge and to continue assisting dependent people considering it “high-priority politics” in their government calendars.
