Amaç: Cerrahi menopoz hastalarında, östrojen formlarından transdermal östradiol hemihidrat jel ve intranasal östradiol hemihidrat sprey uygulamalarının serum hormon, lipid değerleri ve mamografik dansite üzerine etkilerinin karşılaştırılması. Gereç ve Yöntem: Çalışmaya Şişli Etfal Eğitim ve Araştırma Hastanesi Kadın Hastalıkları ve Doğum Kliniği menopoz polikliniğine başvuran, cerrahi menopozlu toplam 58 hasta alındı. Hastalar 3 gruba ayrılarak birinci gruba (n=20) Estrava transdermal jel (Östradiol hemihydrate 3x0,5 mg/gün), ikinci gruba (n=20) ise intranazal sprey (Östradiol hemihydrate 2x150 mcg/gün) nasal sprey verildi. Üçüncü grup (n=18) ise kontrol grubu olarak belirlendi. Hastaların tedavi öncesi ve sonrası mamografik dansitelerindeki değişiklikler Wolfe skorlamasına göre değerlendirildi. Total kolesterol, HDL, LDL-kolesterol, trigliserid, FSH, LH, E2, TSH, testosteron, kortizol ve DHEAS düzeyleri de aynı dönemlerde bakıldı. Bulgular: Transdermal jel, intranazal sprey gruplarının, tedavi sonrası E2 değerleri tedavi öncesinden anlamlı olarak yüksek bulundu (p= 0,003, p=0,0001). Transdermal jel, intranazal sprey, kontrol gruplarının tedavi öncesi ve sonrası mamografik dansiteleri arasında, istatistiksel farklılık gözlenmedi (p>0,05). Sonuç: Çalışmamızda transdermal jel ve intranazal spreyin her ikisinin de serum E2 düzeylerini arttırdığı saptanmış, mamografik değerlendirmede her iki grupta da kontrol grubuna göre dansitede anlamlı fark saptanmamıştır.
Purpose: Comparing the effects of transdermal estradiol hemihydrate gel and intranasal estradiol hemihydrate spray applications on serum hormone, lipid values and mammographic dancity in surgical menopause patients from estrogen forms. Tools and Methods: A total of 58 patients with a surgical menopause, who apply to the Women's Diseases and Birth Clinic, were taken to study in the Bottle Etfal Education and Research Hospital. Patients were divided into 3 groups and given the first group (n=20) Estrava transdermal gel (Estradiol hemihydrate 3x0,5 mg/day), and the second group (n=20) intranazal spray (Estradiol hemihydrate 2x150 mcg/day) nasal spray. The third group (n=18) was defined as the control group. Changes in patients' mammographic dances before and after treatment were assessed according to Wolfe scores. Total cholesterol, HDL, LDL-cholesterol, triglycerides, FSH, LH, E2, TSH, testosterone, cortisol and DHEAS levels were also considered in the same periods. Results: Transdermal gel, intranazal spray groups, after treatment E2 values were found significantly higher than before treatment (p = 0,003, p = 0,0001). There was no statistical difference between transdermal gel, intranazal spray, pre- and post-treatment mammographic dancers of control groups (p>0,05). The result: In our study, transdermal gel and intranasal spray both were found to increase the levels of serum E2, and in the mammographic assessment, there was no significant difference in dancitis according to the control group in both groups.
