Gastroözefageal reflü (GÖR) gastrik içeriğin özefagus içerisine geri kaçışı olarak tanımlanmaktadır. Ebeveynlerin anksiyetesine ve bebeklerin gereksiz yere birçok kez muayene olmasına yol açan sık görülen bir semptomlar kompleksidir. Bebeklik çağında ve tüm yaş gruplarında beslenme sonrası sıkça görülen ve genellikle yaşamın ilk 6-12 ayında kendiliğinden düzelen fizyolojik bir durumdur. İki yaş altındaki sağlıklı bebeklerde fizyolojik GÖR genellikle semptomatik olup rejurjitasyon karakteristik bulgusudur. Bir çok bebekte tekrarlayan rejurjitasyon dışında dikkat çekici bir bulgu yoktur. Eğer bebekte yeterli kilo alımı ve beslenme süreci varsa, anormal bir huzursuzluk hali gözlenmiyorsa bu bebek komplike olmayan GER olarak değerlendirilir. Gastroözefageal reflü hastalığı (GÖRH) ise yetersiz kilo artışı, özefajit ve persiste eden solunum sistemi bulgularının eşlik ettiği patolojik bir klinik tablodur. Tanısal yaklaşımda bir çok tanı metodu kullanılmakla birlikte etkili bir yöntem tanımlanamamıştır. Gastroözefageal reflü hastalığı düşünüldüğünde ilk yapılması gereken hastanın ayrıntılı anamnez ve fizik muayenenin tamamlanmasıdır. Özefageal pH monitorizasyonu, radyolojik ve endoskopik incelemeler gibi tanı metodlarına nadiren başvurulur. Konservatif yaklaşımlar kıvamlı beslenme, pozisyonel beslenme şekli, beslenmeden süt, soya gibi öğelerin çıkarılması ve ebeveynlerin mevcut problem konusunda bilinçlendirilmelerini içermektedir. Birçok hastada ileri tetkike ve antasit/alginat, H2 reseptör antagonisti, proton pompa inhibitörü veya bu tedavi unsurlarının kombinasyonuna gerek duyulmamaktadır. Cerrahi tedaviye çok nadiren başvurulur. Bununla birlikte bebeklik döneminden itibaren sebat eden semptomları olan ve GÖRH tanısı ile izlenen hastalara çocukluk yaş grubunda cerrahi tedavi gerekebilir. Bu derleme yazısında GÖR/GÖRH’nin yenidoğan ve bebeklik dönemindeki klinik bulguları, patofizyolojisi, tanısı ve yaklaşımı güncellenen literatür bilgileri eşliğinde tartışılmıştır. Gastroözefageal reflux (GÖR) is defined as a reflux into the ozephasis of the stomach content. It is a complex of common symptoms that lead to parents' anxiety and the unnecessary examination of babies many times. It is a physiological condition that is frequently observed after eating in childhood and in all age groups and is usually self-repared in the first 6-12 months of life. In healthy babies under two years of age, the physiological look is usually symptomatic and rejury is a characteristic conclusion. There is no remarkable finding in many babies except repeated rejury. If the baby has enough weight gain and the process of nutrition, an abnormal discomfort is not observed, this baby is considered to be uncomplicated GER. Gastroözefageal reflus disease (GÖRH) is a pathological clinical picture that is accompanied by insufficient weight gain, selfie and persistent findings of the respiratory system. Many methods of diagnosis have been used in the diagnostic approach, but a effective method has not been identified. When considering Gastroözefageal reflux disease, the first thing to do is complete the patient's detailed history and physical examination. Diagnostic methods such as ozephageal pH monitoring, radiological and endoscopic examinations are rarely applied. Conservative approaches include poor nutrition, a positional diet, the removal of elements such as milk, soya from the diet, and the awareness of the parents about the current problem. Many patients do not require advanced examination and anthaside/alginate, H2 receptor antagonist, proton pump inhibitors or a combination of these therapeutic elements. Surgery is rarely applied. However, patients with complicated symptoms from childhood and monitored with GÖRH diagnosis may need surgery in childhood age group. In this collection, the clinical findings of GÖR/GÖRH in the newborn and childhood period, pathophysology, diagnosis and approach are discussed along with updated literary information.
