Yoğun Bakım Hemşirelerinin Santral Venöz Kateter İlişkili Enfeksiyonları Önlemede Kanıta Dayalı Kılavuz Bilgilerini Değerlendirme Testinin Türkçe’ye Uyarlanması

ÖZET Amaç: Bu araştırma, Yoğun Bakım Hemşirelerinin Santral Venöz Kateter İlişkili Enfeksiyonları Önlemede Kanıta Dayalı Kılavuz Bilgilerini Değerlendirme Testini Türkçe’ye uyarlamak amacıyla gerçekleştirilmiştir. Gereç ve Yöntem: Araştırma, bir eğitim ve araştırma hastanesinin bir yoğun bakım ünitesinde, Aralık 2015-Ocak 2016 tarihleri arasında yürütülmüştür. Klinik (yatak başı) hemşiresi olan, en az bir aydır ünitede çalışan ve araştırmaya katılmaya gönüllü olan 50 yoğun bakım hemşiresi araştırmaya alınmıştır. Verilerin değerlendirilmesinde ortalama ve standart sapma, sayı ve yüzde, Kendall’s W iyi uyuşum analizi ve Pearson korelasyon analizi kullanılmıştır. Test sorularının kapsam geçerlik indeksi değerlendirilmiş ve madde analizleri yapılmıştır. Bulgular: Her iki dili iyi bilen uzmanlar tarafından geri çeviri ve uzman görüşleri doğrultusunda dil/ ifadeye yönelik önerilen düzeltmeler yapılmıştır. Tüm soru ve yanıt seçenekleri ve toplam bilgi testi için kapsam geçerlik indeksi değeri 1.00 olarak bulunmuştur. Testin kapsam geçerliği olduğu gösterilmiştir. Uzman görüşleri arasındaki uyum analiz edilmiş ve uyumun olduğu görülmüştür (Kendall’s W=.111, p=.473). Eş zamanlı ölçek geçerliği incelenmiş, hemşirelerin bilgi testi ile tutum ölçeği puanları arasında anlamlı bir ilişki olduğu bulunmuştur (r=.61, p<.001). Bilgi testinin ölçüt geçerliğinin olduğu saptanmıştır. Madde analizlerinde, madde güçlük indeksi, madde ayırıcılık gücü indeksi ve çeldirici seçenek analizi sonuçlarının uygun düzeyde olduğu belirlenmiştir. Sonuç: Sonuçlar, Yoğun Bakım Hemşirelerinin Santral Venöz Kateter İlişkili Enfeksiyonları Önlemede Kanıta Dayalı Kılavuz Bilgilerini Değerlendirme Testinin kapsam ve ölçüt geçerliği olduğunu göstermiştir. Test sorularının madde analizi sonuçlarının uygun düzeyde olduğu ortaya koyulmuştur. Testteki doğru yanıtların kanıta dayalı kılavuzlar güncellendikçe gözden geçirilmesi ve testin psikolinguistik ve psikometrik özelliklerinin farklı örneklemlerde test edilmesi önerilmiştir. ABSTRACT Objective: This study was conducted for the purpose of adapting the Evidence-Based Guideline Knowledge Assessment Test of Critical Care Nurses in Preventing Central Venous Catheter Related Infections into Turkish. Material and Method: The study was carried out in the intensive care unit of a training and research hospital between December 2015-January 2016. Fifty intensive care nurses who were clinic (bedside) nurses, had been working in the unit for at least one month and volunteered to participate, were recruited to the study. Data were evaluated using mean and standard deviation, number and percentage, Kendall’s W concordance analysis and Pearson’s correlation analysis. Content validity of the test questions was assessed and item analyses were performed. Findings: Corrections offered for language/expressions were made by experts speaking both languages very well, in accordance with back translation and expert opinions. Content validity index value was found to be 1.00 for all question and answer options, as well as total information test. The test was demonstrated to have a content validity. Concordance among expert opinions was analyzed and proved to be available (Kendall’s W=.111, p=.473). Concurrent scale validity was examined and it was found that there was a significant correlation between the nurses’ information test and attitude scale scores (r=.61, p<.001). It was determined that information test had a criterion validity. In the item analyses it was determined that item difficulty index, item discrimination power index and distracter option analysis results were at a convenient level. Conclusion: The results have shown that Evidence-Based Guideline Knowledge Assessment Test of Critical Care Nurses in Preventing Central Venous Catheter Related Infections had content and criterion validity. It was revealed that item analysis results of the test questions were at a convenient level. It was recommended to review correct answers in the test whenever evidence-based guidelines would be updated and to test psycholinguistic and psychometric features of the test in different samples. This study was carried out with the aim of adapting the Intense Nursing Nurses’ Evidence-Based Guidelines for Prevention of Infections Related to Central Venus Catheter to the Turkish Test. Tools and Methods: The research was carried out in an intensive care unit of an educational and research hospital, between December 2015-January 2016. 50 intensive care nurses, who have been clinical nurses, who have worked in the unit for a few months and are volunteer to participate in the research, have been surveyed. In the assessment of the data, the average and standard deviation, the number and percentage, Kendall's W's good consistency analysis and Pearson's correlation analysis were used. The scope of the test questions is validity index assessed and the material analysis is carried out. Results: The recommended language/expression corrections have been made by experts who know both languages well in accordance with the feedback and expert opinions. For all questions and answers options and the total information test, the scope validity index value is 1.00. The test has shown its validity. The consistency between the experts has been analyzed and the consistency has been shown (Kendall's W=.111, p=.473). The validity of the simultaneous scale has been studied and it has been found that there is a meaningful relationship between the information test and the attitude scale points (r=.61, p<.001). The data test is valid. In substance analysis, the substance difficulty index, the substance distinctive strength index and the results of the analytics of the scattering option were determined to be at an adequate level. Results: The results showed that intensive care nurses have the scope and measurement validity of the test to assess evidence-based guidance information in the prevention of central venous catheter-related infections. The test questions have shown that the material analysis results are at an appropriate level. The correct answers in the test were recommended to be reviewed as the evidence-based guidelines are updated and the psycholinguistic and psychometric characteristics of the test should be tested in different samples.
