Adli Otopsilerde Santral Sinir Sistemi Enfeksiyonu Saptanan Olguların Değerlendirilmesi

Abstract:

Amaç Çalışmada amacımız, retrospektif olarak santral sinir sistemi enfeksiyonlarının histopatolojik özelliklerini literatür bilgileri ışığında değerlendirmektir. Gereç- Yöntem 1998-2002 yıllarında Adli Tıp Kurumu Morg ihtisas Dairesinde otopsileri yapılan ve santral sinir sistemi enfeksiyonu saptanan 103 olgu çalışmaya alındı. Olgularımız yaş, cinsiyet, ölüm nedenleri (doğal ve travmaya bağlı ölümler) ve beyinde saptanan enfeksiyonun histopatolojik özellikleri açısından değerlendirildi. Bulgular Çalışmaya alınan 103 olgunun %50,4’ü doğal, %49,6’sı travmaya bağlı gelişen komplikasyonlar sonucunda ölen olgulardı. Olgularımızın 85’i (%83,3) erkek, 18’i (%16,6) kadındı. Histopatolojik olarak, toplam 73 olguda (%70,87) menenjit saptandı. Bu grupta 60 olgu (%58,25) akut piyojenik (bakteryel) menenjit, 8 olgu (%7,76) akut aseptik (viral) menenjit, 5 olgu (%4,85) granülomatöz menenjit olarak değerlendirildi. Granülomatöz menenjit olgularından birinde (%20) beyin sapı tutulumunun da eşlik ettiği nokardia enfeksiyonu, dördünde (%80) tüberküloz enfeksiyonu saptandı. 19 olguda (%18,44)ensefalit saptandı. Bu grupta 8 olgu nonspesifik ensefalit (%7,76), 5 olgu (%4,85) kuduza bağlı viral ensefalit, 1 olgu (%0,97) HIV’e bağlı ensefa-lit, 1 olgu (%0,97) progresif mültifokal lökoensefalopati, 4 olgu (%3,88) Prion hastalığı (Jacob-Creutzfeld) olarak değerlendirildi. Serimizde 3 olguda (%2,91) meningoensefalit , 6 olguda (%5,82) beyin absesi, 2 olguda da (%1,94) non-spesifik serebrit tespit edildi. Sonuç Santral sinir sisteminde enfeksiyon hastalıklarının patolojisi oldukça karmaşık ve sıklıkla birbirleriyle örtüşen tabloları içermektedir. Sonuç olarak, enfeksiyon hastalıklarının patolojisinde mültidisipliner bir laboratuar araştırma yapılması gerekmektedir.

Abstract:

Our aim in the study is to retrospectively evaluate the histopathological characteristics of central nervous system infections in the light of literary information. In the years 1998-2002 the Court Medical Institute of Morg specialized department of the autopsy and detected the central nervous system infection 103 cases were taken to study. Our cases have been evaluated in terms of age, gender, causes of death (natural and traumatic deaths) and the histopathological characteristics of the infection detected in the brain. The study found that 50.4 percent of the 103 incidents were natural and 49.6 percent were dead as a result of developing trauma-related complications. 85 of our cases (83.3%) were men and 18 (16.6%) women. Histopathologically, menenjitis was detected in a total of 73 cases (70.87) . In this group, 60 cases (58.25%) were considered acute pyogenic (bacterial) menenjitis, 8 cases (7.76) acute aseptic (viral) menenjitis, 5 cases (4.85) granulomatosis menenjitis. In one of the occurrences of granulomatosis menenjitis (%20) the nocardial infection, which is also accompanied by brain disconnection, in four (%80) tuberculosis infection was detected. In 19 cases (18.44) the infection was found. In this group, 8 incidents were assessed as non-specific encephalitis (7.76%), 5 incidents (4.85) viral encephalitis associated with gonorrhea, 1 incidence (0.97) encephalitis associated with HIV, 1 incidence (0.97) progressive proprietary lokoencephalopathy, 4 incidents (3.88) Prion disease (Jacob-Creutzfeld). In our series, 3 cases (2.91) meningoensephalitis, 6 cases (5.82) brain abscess, and 2 cases (1.94) non-specific cerebritis were detected. The pathology of infectious diseases in the central nervous system is quite complex and often contains tables covered with each other. As a result, a multidisciplinary laboratory research in the pathology of infectious diseases is necessary.
