Amaç: Bu çalışmada, erken başlangıçlı şizofreni (EBŞ) tanılı olguların Döner Kapı Fenomeni (RDP) kapsamında değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Türkiye’de üç merkeze ait çocuk ve ergen psikiyatrisi yataklı servis verileri geriye dönük olarak incelenerek; olguların sosyo-demografik özellikleri, psikiyatrik eş tanıları, tedavi modaliteleri, tedavi öncesi ve sonrası değerlendirmeleri uygulanan Pozitif ve Negatif Sendrom Ölçeği (PANSS) ve Çocuklar için Global Değerlendirme Ölçeği (CGAS) puanları ile değerlendirilmiştir. Çalışmaya dahil edildiği yatışla beraber 18-60 ay takip edilen olgular içinde tekrarlayıcı olan hastalar değerlendirilerek, iki ve üzeri yatışı olan hastalar RDP (+) olarak kabul edilmiştir. Bulgular: RDP (+) (n=40) ve RDP (-) (n=35) grupları arasında sosyodemografik özellikler açısından sadece madde kullanım bozukluğu öyküsü (p=0,007) karşılaştırıldığında anlamlı fark saptandı. İki grup arasında yatışları sırasında kullanılan psikotrop ilaçlar karşılaştırıldığında haloperidol (p=0,001) ve Birinci Kuşak Antipsikotikler (BKA) (p=0,011) açısından farklılıklar saptanmıştır. Sonuçlar, toplam PANSS yatış (p=0,044) ve CGAS yatış skorları (p=0,006) için tanımlanan 18-60 ay takibi olan hastaların yatış sayısı arasında anlamlı bir etkileşim olduğunu saptadı. Yapılan regresyon analizlerinde madde kullanım bozukluğu öyküsü, BKA kullanımı ve yatış toplam PANSS puanlarının EBŞ tanılı olgularda RDP için anlamlı düzeyde yordayıcı faktörler olduğu bulunmuştur. Sonuç: Çalışmamızda, belirli semptomatolojik boyutların erken tanımlanmasına, madde kullanım bozukluğu öyküsünün değerlendirilmesi ve olguların tedavi yönetimine odaklanan hedefli stratejilerin RDP’yi azaltmaya yardımcı olabileceğini düşündürmektedir.
Purpose: In this study, it is intended to evaluate the events identified in early schizophrenia (EPS) within the framework of the Reverse Door Phenomenon (RDP). Tools and Methods: In Turkey, three centres of children and adolescent psychiatric bed service data have been reviewed backward; the socio-demographic characteristics of the incidents, psychiatric co-diagnosis, methods of treatment, the positive and negative syndrome measurement (PANSS) and the global assessment measurement for children (CGAS) scores are applied. Patients with twice and over twice and over twice and over twice and over twice and over twice and over twice and over twice and over twice and over twice and over twice and over twice and over twice and over twice and over twice. Results: A significant difference between the RDP (+) (n=40) and the RDP (-) (n=35) groups was found in terms of sociodemographic characteristics when compared only the history of substance use disorders (p=0,007). When compared psychotropic medications used during treatment between the two groups, differences were found in haloperidol (p=0,001) and First Generation Antipsychotics (BKA) (p=0,011). The results found that there was a meaningful interaction between the number of patients with 18-60-month trace defined for the total PANSS deposit (p=0,044) and the CGAS deposit scores (p=0,006). Regression analysis has found that the history of substance use disorders, the use of BKA and the total PANSS points of the investment have been significantly contributing factors for RDP in the EBŞ-recognized facts. The result: Our study suggests that early identification of certain symptomatological dimensions, evaluation of the history of disorder use of the substance and targeted strategies focused on the treatment management of the events can help reduce the RDP.
