Amaç: Üriner sistem enfeksiyonları (ÜSE), klinikte en sık karşılaşılan toplum kökenli bakteriyel enfeksiyonlardır. Bu çalışmada, toplum kökenli ÜSE etkenlerinden en sık izole edilen Escherichia coli suşlarının özellikle ampirik tedavide sık kullanılan antibiyotiklere karşı direnç durumlarının araştırılması amaçlanmıştır. Gereç ve Yöntem: Toplum kökenli ÜSE tanısı konulan hastalardan uygun şartlarda alınan idrar kültürleri Fırat Üniversitesi Hastanesi Enfeksiyon Hastalıkları Laboratuarı'na gönderildi. Son bir yıl içerisinde enfeksiyon etkeni olarak izole edilen 222 E. coli suşu çalışmaya dahil edildi. İdrar örnekleri %5 koyun kanlı agar ve Eosin Metilen Blue agar besiyerlerine ekilerek 37°C'de 18-24 saat inkübe edildi. İdrarda >105 cfu/mL üreme olması pozitif sonuç olarak kabul edildi. Suşların identifikasyonunda konvansiyonel yöntemler kullanıldı. İzolatların antibiyotik duyarlılıkları Clinical and Laboratory Standards Institute kriterlerine göre Kirby-Bauer disk difüzyon yöntemi ile araştırıldı. Bulgular: Çalışmada E. coli suşlarına karşı en etkili antibiyotikler sırasıyla; fosfomisin, nitrofurantoin ve amikasin bulundu. Direnç oranı en yüksek olanlar ise sırasıyla; ampisilin, trimetoprim-sulfametoksazol ve seftriakson idi. Sonuç: Toplum kökenli enfeksiyonlarda ampirik tedavide kullanılacak antibiyotikleri seçerken ülkemizde hatta bölgemizde direnç oranlarını iyi bilmemiz gereklidir. Bunun için mikroorganizmaların, özellikle sık karşılaşılan etkenlerin yerel duyarlılıklarının belirli dönemlerde araştırılması önem arz etmektedir.
Purpose: Urinary system infections (USE) are the most common in the clinic community-based bacterial infections. This study aims to investigate the conditions of resistance of the Escherichia coli juices, which are most often isolated from the community-based ACE factors, especially against antibiotics that are often used in empiric therapy. Tools and methods: Urine cultures taken in appropriate conditions from patients diagnosed with a community-based USE have been sent to the Infectious Diseases Laboratory of the University Hospital of Fırat. In the last year, 222 E. coli juices, which were isolated as an infection agent, were included in the study. The urine samples were planted in 5% cattle blood agar and Eosin methylene blue agar foods and incubated at 37°C for 18-24 hours. Reproduction in the urine >105 cfu/mL was considered a positive result. Conventional methods were used in the identification of the crimes. The antibiotic sensitivity of isolates was studied by the Kirby-Bauer disc diffusion method according to the Clinical and Laboratory Standards Institute criteria. Results: The study found the most effective antibiotics against E. coli juices, respectively; phosphomycin, nitrofurantoin and amikasin. The highest resistance rates were, respectively, ampisilin, trimetoprim-sulfametoxazole and seftriaksone. The result: When choosing antibiotics to be used in empiric treatment in community-based infections, it is necessary that we know well the resistance rates in our country and even in our region. For this, it is important to investigate the local sensitivities of microorganisms, especially frequently encountered factors, in certain periods.
