Pilomatrixoma is a benign calcified adnexal tumor originating from pluripotent precursor matrix cells of hair follicles. It generally arises as a deep subcutaneous or a superficial nodule localized in the head and neck region or upper extremities. It is frequently seen in the first two decades of life and slightly more common in females than in males. Tumoral masses comprise a group of local causes that give rise to carpal tunnel syndrome. In this case we report; the mass lesion localized in the carpal tunnel producing median nerve compression signs was totally excised and delineated as pilomatrixoma after pathologic examination. No case of pilomatrixoma localized in the carpal tunnel and causing carpal tunnel syndrome was met in the literature.
