Amaç: Göğüs duvarı tümörlerinin cerrahisi ile ilişkili çalışmalar ağırlıklı olarak rekonstrüksiyon yöntemlerinin etkinliğine yöneliktir. Bu çalışmanın amacı, sadece rekonstrüksiyon yapılan olgularda değil, rekonstrüksiyon yapılsın veya yapılmasın toraks duvar rezeksiyonu (TDR) sonrası, oluşan komplikasyonlara etki eden faktörleri araştırmaktır. Gereç ve Yöntem: Şubat 2009 ve Şubat 2020 arasında TDR uygulanmış 70 olgunun dosyaları incelendi. Etiyoloji (iyi huylu tümörler ve malign tümörler), rekonstrüksiyon yöntemleri ve komorbiditeler gibi risk faktörleri komplikasyonlar açısından analiz edildi. Bulgular: 13 (%18,6) olguya titanyum plakla rekonstrüksiyon yapılırken, 22 (%31,4) olguda mesh kullanılmıştı ve 35 (%50) olgu sentetik materyalle rekonsükte edilmeyip primer kapatılmıştı. Komplikasyonlar toplamda 22 (%31,4) hastada görüldü. Atelektazi 10 (%14,3), pnömoni 9 (%12,9), uzamış hava kaçağı 7 (%10,0), yara yeri sorunları (seroma 3, enfeksiyon 4) 7 (%10,0) hastada gözlendi. . Benign grupta 2(%11,1) hastada, primer malignitelilerde 3(%33,3), metastatik grupta 3(%18,8) ve invazyon grubunda 14(%51,9) olguda komplikasyon oluşmuştu (p=0,04). Tek değişkenli analizde posterior yerleşim (p=0,03), yüksek ASA skoru (p=0,02), eşlik eden akciğer rezeksiyonu (p=0,04), mesh ile onarım veya rekonstrüksiyon yapmama (p=0,05) komplikasyonlar için risk faktörüydü. Çok değişkenli analizde titanyum plakla rekonstrüksiyonlarda daha düşük komplikasyon izlenmişti (p=0,08). Eşlik eden akciğer cerrahisinin kötü risk faktörü olduğunu gösterildi (p=0,04), (Hazard ratio=5,4(1,1-25,9), %95 GI). Sonuç: Bu çalışmada titanyum plaklarla yapılan rekonstrüksiyonlarda daha düşük komplikasyon oranı izlenirken, eş zamanlı akciğer rezeksiyonu yapılanlarda artmış komplikasyon oranı saptanmıştır. TDR majör bir cerrahi girişimdir, uygun rekonstrüktif yöntem seçimi komplikasyonlar üzerine doğrudan etkili bir faktör gibi görünmektedir.
Purpose: Studies related to the surgery of breast wall tumors are mainly aimed at the effectiveness of reconstruction methods. The purpose of this study is to investigate the factors that affect the complications that occur, whether or not the reconstruction is done or not, after torace wall resection (TDR). Tools and Methods: The files of 70 events applied to TDR between February 2009 and February 2020 have been examined. Risk factors such as etiology (good-fashioned tumors and malignant tumors), reconstruction methods and comorbidities were analyzed in terms of complications. Results: 13 (%18.6) incidents were reconstructed with titanium plate, 22 (%31.4) incidents were used in mesh and 35 (%50) incidents were not reconstructed with synthetic material and the primary was closed. Complications were observed in a total of 22 (31.4%) patients. Atelectase 10 (%14,3), pneumonia 9 (%12,9), prolonged air leakage 7 (%10,0), wound locations problems (seroma 3, infection 4) 7 (%10,0) were observed in patients. in the Benign group 2(%11,1) patients, in primary malignities 3(%33,3), in metastatic group 3(%18,8) and in the invasion group 14(%51,9) complications were observed (p=0,04). In one-variable analysis, posterior placement (p=0,03), high ASA score (p=0,02), accompanying lung resection (p=0,04), repair or non-rebuilding (p=0,05) were a risk factor for complications. In multiple variable analyses, lower complications were observed in titan plaque reconstructions (p=0,08). It was shown that accompanying lung surgery was a bad risk factor (p=0,04), (Hazard ratio=5,4(1,1-25,9), 95% GI). The result: In this study, a lower rate of complications was observed in reconstructions with titanium plates, while an increased rate of complications was observed in simultaneous lung resection. The major TDR is a surgical initiative, the choice of the appropriate reconstructive method appears to be a direct-effective factor on complications.
