Amaç: Çocuk ve ergenlerde saptanan özkıyım girişimleri tüm dünyada etkisini gösteren bir halk sağlığı sorunudur. Madde kullanım bozukluğu özkıyım girişimlerine sıklıkla eşlik eden psikiyatrik bozukluklardandır. Bu çalışmadaki amacımız, madde kullanımı olan çocuk ve ergenlerde özkıyım girişimi bulunan ve bulunmayanlar arasındaki sosyodemografik farklılıkların belirlenmesidir. Gereç ve Yöntem: Bu çalışma kapsamında Ege Üniversitesi Çocuk ve Ergen Bağımlılık Merkezi’ne (EGEBAM) poliklinik ya da denetimli serbestlik değerlendirmesi amacıyla başvuran çocuk ve ergenlerin arşiv dosyaları geriye dönük olarak incelenmiştir. Olguların özkıyım girişiminin olup olmadığı, sigara ya da madde kullanımına başlama yaşları, aile öyküleri, önceki tedavi öyküsü, sokakta yaşama öyküsü, adli sorun yaşama öyküsü gibi değişkenler değerlendirilmiştir. Bulgular: EGEBAM Polikliniği'ne başvurmuş olup dosyaları incelenen 371 olgunun %8’inde özkıyım girişimi öyküsü mevcuttu. Özkıyım girişimi ile istatistiksel olarak anlamlı düzeyde ilişkili olan değişkenler şu şekildeydi: Kız cinsiyet, önceki tedavi öyküsünün varlığı, ailede madde kullanımı öyküsünün varlığı ve sokak yaşantısı öyküsünün varlığı (p<0,001). Olguların özkıyım girişiminden bağımsız şekilde sigara kullanımına başlama yaşı ile madde kullanımına başlama yaşı arasında yaklaşık 2 yıl bulunmaktaydı. Sonuç: Özkıyım ile ilişkili risk faktörlerinin belirlenmesi erken müdahalelerin yapılması açısından önem taşımaktadır. Özellikle, kız cinsiyete sahip, geçmiş tedavi öyküsü bulunan, ailede madde kullanımı olan ve hayatının bir döneminde sokakta yaşama öyküsü olan olgular özkıyım açısından daha riskli olmaları nedeni ile daha detaylı bir değerlendirme bu riskin azaltılmasına olanak sağlayabilecektir. Bununla birlikte, çocuk ve ergen psikiyatrisinde acil müdahale gerektiren madde kullanımı ve özkıyım ilişkisini ve seyrini araştıran daha çok sayıda çalışmaya ihtiyaç duyulmaktadır.
Purpose: Child and adolescent self-sufficiency initiatives are a public health issue that affects the worldwide. Substantial use disorder is a psychiatric disorder that is often accompanied by the attempts of self-determination. Our purpose in this study is to identify the socio-demographic differences between those who have and do not have an attempt to self-determinate in children and adolescents using the substance. Tools and Methods: In this study, the archive files of children and adolescents applying for a clinical or controlled freedom assessment to the Ege University Child and Adolescent Addiction Center (EGEBAM) have been reviewed backward. It has been assessed whether the events are self-sufficient initiative, variables such as the age of starting to smoke or use of the substance, family stories, previous treatment history, history of living in the street, history of life of legal problems. Results: 8% of the 371 cases examined and applied to the EGEBAM Clinic had a history of self-determination. The variables that were statistically significantly linked to the initiative of self-determination were as follows: the girl’s gender, the presence of the previous history of treatment, the presence of the history of the use of the substance in the family and the presence of the history of the street life (p<0,001). The age between the beginning of smoking and the beginning of the use of the substance was about 2 years. Result: Identification of risk factors associated with self-determination is important for early interventions. In particular, a more detailed assessment of the facts with a girl's sex, having a history of treatment, having a family use of substances and having a history of living in the street during a period of her life, with the reason that they are more risky in terms of self-determination, will allow this risk to be reduced. However, more studies are needed to investigate the relationship and course of the use of the substance that requires urgent intervention in child and adolescent psychiatry.
