AIM: We aimed to compare the short term outcomes of off-pump and on-pump (cardiopulmonary bypass) coronary revascularization surgeries in diabetic patients. METHODS: In this retrospective study, we analyzed the pre, intra and post operative characteristics of 138 diabetic patients underwent isolated coronary artery bypass grefting between January 2002 and December 2008. The patients were allocated into two groups. Group 1 consisted of 112 (81.2%) patients operated using the off off-pump coronary procedures and Group 2 consisted of 26 (18.8%) patients operated using the on-pump coronary procedures. RESULTS: Preoperative prognostic factors were similar in both groups. However, off-pump patients received signifi cantly fewer distal anastomoses than the on-pump group (2.7±0.7 versus 3.1±0.69; P< 0.05). Intra and post operative mortality rates were similar in both groups (off-pump 2.7%, on-pump 3.8%; p> 0.05), however the off-pump group had signifi cantly fewer neurological complications (0.9% versus 15.3%; p< 0.05). Off-pump patients also required less blood transfusion and had shorter lengths of hospital stay (p< 0.05). CONCLUSION: The off-pump coronary operation in diabetic patients signifi cantly reduces post operative morbidity and length of hospital stay compared with the on-pump coronary operation. However, in-hospital survival rates are similar for both techniques.
