Tip 2 diabetes mellituslu hastalarda yaşam tarzı değişikliklerine uyum

Abstract:

Amaç: Kliniğimizde takip edilen tip 2 diyabetes mellitus (DM) tanılı hastaların yaşam tarzları, önerilen değişiklikler ve hastaların bu önerilere uyumu araştırıldı. Hastalar ve yöntemler: İzmir Tepecik Eğitim ve Araştırma Hastanesi İç Hastalıkları Kliniği’nde 1 Haziran - 30 Ağustos 2015 tarihleri arasında yatarak takip edilen veya polikliniğe başvuran tip 2 DM tanılı 160 hastaya (59 erkek, 101 kadın; ort. yaş 52.6±9.4 yıl), tarafımızca geliştirilen “Diyabet Yaşam Tarzı Anketi” bire bir görüşme yöntemiyle uygulandı. Bulgular: Ortalama vücut kütle indeksi erkekler için 29.2±6.4, kadınlar için 33.5±13.0 idi. Tüm hastaların %30’u sadece oral anti diyabetik (OAD) tedavisi, %33.8’i sadece insülin tedavisi, %36.2’si ise her iki tedaviyi birlikte alıyordu. Hastaların ortalama HbA1c değeri 9.8±2.2, HDL değeri 40.3±1.2 mg/dL, LDL değeri 107.3±5.0 mg/dL ve trigliserit değeri 201.8±9.3 mg/dL olarak saptandı. Kardiyovasküler hastalık oranı %33.1 (n=53), hipertansiyon %52.5 (n=84), retinopati %40.6 (n=65), nöropati %25 (n=40), diyabetik ayak ülseri %11.9 (n=19) ve nefropati %15.6 (n=25) idi. Yetmiş dokuz hasta daha önce diyabet hakkında eğitim almıştı. Hastaların %28.8’i tıbbi beslenme tedavisini tam olarak uyguluyordu. Ancak %32.5’inin düzensiz olarak uyguladığı, %32.5’inin ise hiç uygulamadığı belirlendi. Hastaların %11.2’si her gün, %10.0’u haftada üç veya dört gün, %8.8’i haftada bir veya iki gün, %2.5’i iki haftada bir gün egzersiz yaparken, %15.6’sı hiç egzersiz yapmıyordu. Sonuç: Hastalarımız yaşam tarzı değişikliklerine yeterli uyum sağlamamaktadır. Bunun sağlanması için uygun hasta eğitim ortamları oluşturulmalıdır. Ayrıca hekim, hemşire, diyetisyen, psikolog ve diğer sağlık çalışanları birlikte multidisipliner olarak çalışmalıdır.

Purpose: Patients with type 2 diabetes mellitus (DM) diagnosed in our clinic have been studied in their lifestyle, recommended changes and compliance with these recommendations. Patients and methods: Izmir Reactive Education and Research Hospital in the Clinic of Internal Diseases from 1 June to 30 August 2015 to 160 patients diagnosed with type 2 DM, followed by bed or applying to the clinic (59 men, 101 women; mid. Age 52.6±9.4 years), the "Diabetes Lifestyle Survey" developed by us was implemented by a conversation method. Results: The average body mass index was 29.2±6.4 for men, 33.5±13.0 for women. 30 percent of all patients received oral anti-diabetic (OAD) treatment only, 33.8 percent received insulin treatment only and 36.2 percent received both treatments together. The average HbA1c in patients is 9.8±2. 2, the HDL value was 40.3±1.2 mg/dL, the LDL value was 107.3±5.0 mg/dL and the triglyceride value was 201.8±9.3 mg/dL. The rate of cardiovascular disease was 33.1 % (n=53), hypertension 52.5 % (n=84), retinopathy 40.6 % (n=65), neuropathy 25 % (n=40), diabetic footsteps 11.9 % (n=19) and nefropathy 15.6 % (n=25). Seventy-nine patients had previously been trained on diabetes. 28.8 percent of patients were fully treated with medical nutrition. But it was found that 32.5 percent applied irregularly, and 32.5 percent did not apply anyway. 11.2 percent of patients did exercise every day, 10.0 percent did not exercise every day, three or four days a week, 8.8 percent did exercise one or two days a week, 2.5 percent did not exercise every two weeks, while 15.6 percent did not exercise at all. The result: Our patients are not adequately adapted to lifestyle changes. It is necessary to create appropriate patient training environments to ensure this. Also, doctors, nurses, nutritionists, psychologists and other health workers should work together as multidisciplinary.
