Amaç: Bu çalışmanın amacı pulmoner hipertansiyonlu (PH) bireylerde kinezyofobinin egzersiz kapasitesi, günlük yaşam aktiviteleri (GYA) ve yaşam kalitesi ile ilişkisini araştırmak ve demografik ve klinik özelliklere göre karşılaştırmaktı. Yöntem: Çalışmaya 37 PH’li hasta dahil edildi. Kinezyofobi değerlendirmesi, Kalp İçin Tampa Kinezyofobi Derecelendirmesi (KTKD) ile yapıldı. Yaşam kalitesi, Nottingham Sağlık Profili (NSP) ile belirlendi. Günlük yaşam aktiviteleri, London Chest Günlük Yaşam Aktiviteleri Ölçeği (LCGYAÖ) ile değerlendirildi. Egzersiz kapasitesi, altı dakika yürüme testi (6DYT) ile ölçüldü ve 6DYT mesafesi hesaplandı. Sonuçlar: KTKD toplam skor, 6DYT sonrası dispne algılaması (p=0,016), NSP toplam skoru (p=0,024), NSP alt gruplarından emosyonel reaksiyonlar (p<0,001) ve ağrı (p=0,018) ile pozitif yönlü ilişkili bulundu. KTKD toplam skor ve KTKD alt grup skorları ile yaş, pulmoner arter basıncı, 6DYT mesafesi ve LCGYAÖ arasında anlamlı ilişki yoktu (p>0,05). Çoklu doğrusal regresyon analizinde dispne algılaması (p=0,006) ve emosyonel reaksiyonların (p<0,001) KTKD toplam skorunu etkilediği bulundu (R2=0,439, F(2,34)=13,309, p<0,001). Düşük eğitim seviyesi (p=0,023) ve dispne sebebi ile GYA etkilenimi (p=0,029) olan olgularda KTKD toplam skoru daha yüksek saptandı. Tartışma: Çalışmanın sonuçları PH’li bireylerde, kinezyofobinin eforla algılanan nefes darlığındaki artış ve emosyonel reaksiyonlardan etkilendiğini ve azalmış yaşam kalitesi ile ilişkili olduğunu gösterdi. Ayrıca düşük eğitim seviyesine sahip PH’li bireylerde kinezyofobi düzeyinin arttığı görüldü.
The aim of this study was to investigate the relationship of kinesiophobia with exercise capacity, daily life activities (GYA) and quality of life in individuals with pulmonary hypertension (PH) and to compare it according to demographic and clinical characteristics. Method: 37 PH patients were included in the study. The kinezyofobia assessment was performed with the Tampa Kinezyofobia Rating for the Heart (KTKD). The quality of life was determined by the Nottingham Health Profile (NSP). Daily life activities were assessed by the London Chest Daily Life Activities Scale (LCGYAÖ). Exercise capacity was measured by a six-minute walk test (6DYT) and a distance of 6DYT was calculated. Results: The total score of KTKD, the post-DYT dyspne detection (p=0,016), the total score of NSP (p=0,024), the emotional reactions from the subgroups of NSP (p<0,001) and the pain (p=0,018) were found to be positively related. There was no meaningful relationship between the total score of the KTKD and the sub-group score of the KTKD and age, pulmonary arterial pressure, 6DYT distance and LCGYAÖ (p>0,05). In the multi-linear regression analysis, it was found that dyspne perception (p=0,006) and emotional reactions (p<0,001) affected the total score of KTKD (R2=0,439, F(2,34)=13,309, p<0,001). In cases with low training level (p=0,023) and GYA influence due to dispne (p=0,029) the total score of KTKD was higher. Discussions: The results of the study in individuals with PH showed that kinesiophobia was affected by increased and emotional reactions in stressed breathing and was associated with a decreased quality of life. It was also seen that kinesiophobia levels increased in individuals with low educational level of PH.
