İnflamatuvar bağırsak hastalıkları (İBH), iki temel formuyla Crohn hastalığı (CH) ve Ülseratif Kolit (ÜK) olup gastrointestinal sistemde kronik, tekrarlayıcı inflamasyona neden olurlar. Bu hastalıklar genetik yatkınlık, çevresel ve immünolojik etmenlerin bir araya gelmesi sonucu, gastrointestinal florayı, barsağın tüm katmanlarını etkileyen, uyumsuz immün yanıtların tetiklendiği inflamatuvar olaylar dizisidir. CH ağız, özefagus, mide, bağırsak ve anüse kadar olan bölgede inflamatuvar aktivitelerle mukozayı etkilerken, ÜK yalnızca kolonik mukozayı etkiler. CH’ın etiyopatogenezi tam olarak anlaşılmamakla birlikte çevresel etmenlerden, sigara, enfeksiyon, apendektomi, emzirme süresi, beslenme alışkanlıkları, ilaçlar, sosyoekonomik etmenler ve stresle ilişkilidir. Bağışıklık sistemde önemli rol oynayan gen mutasyonunun otoimmün hastalıklara ve İBH’a neden olduğu bilinmektedir. Genellikle 15-30 yaş arası görülen CH, fistül, bağırsak tıkanıklığı ve karın içi abse gibi ciddi komplikasyonlar oluşturmaktadır. Hastaneye yatan CH’da %75 beslenme yetersizliği ve ağır mikrobesin ögesi kayıpları görülür, özellikle emilim bozukluğu, intestinal kayıplar, besin metabolizmasındaki değişiklikler, inflamasyon şiddeti, bulantı, kusma, karın ağrısına bağlı tüketimin azalması/gereksinmenin artması, steroidlerin vitamin ve minerallere olumsuz etkisi, GI’in tutulum yeri, hastalıklı barsağın uzunluğu, rezeke edilmesi ve yine hastalığın alevlenme süresi/sıklığı, beslenme yetersizliğine, protein, vitamin, mineral, eser element kayıplarına neden olmaktadır. CH’ın beslenme tedavisi remisyon ve aktif döneme, gelişen komplikasyonlara göre, cerrahi öncesi ve sonrasında değişkenlik arz eder ve dinamik bir süreçtir. Beslenme tedavisinin amacı atakları azaltmak, aktif dönemde vücut bileşimini korumak, makro ve mikrobesin ögeleri kayıplarını önlemek ve yerine koymak, yaşam kalitesini arttırmak olmalıdır. Bu makalede CH tanısı alan bir hastanın gelişen komplikasyonlara göre farklı beslenme tedavi stratejilerinin gerçekleştirildiği olgu tartışılmaktadır.
Inflammatory intestinal diseases (IBH) are two main forms of Crohn's disease (CH) and Ulcerative Colitis (UC) which cause chronic, repetitive inflammation in the gastrointestinal system. These diseases are a series of inflammatory events resulting from a combination of genetic tendency, environmental and immune factors, affecting the gastrointestinal flora, all layers of the bark, in which incompatible immune responses are triggered. While CH affects the mucous with inflammatory activities in the area of the mouth, ozephasis, stomach, intestine and anus, the UFO affects only the colonic mucous. Although the etiopathogenesis of CH is not fully understood, it is associated with environmental factors, smoking, infection, apendectomy, breastfeeding time, eating habits, medications, socioeconomic factors and stress. It is known that genetic mutations that play an important role in the immune system cause autoimmune diseases and IBH. Usually between the ages of 15-30, CH causes serious complications such as fistulate, intestinal obstruction and intra-abse. 75% nutritional insufficiency and severe micronutrient loss in the hospital CH occur, especially emulation disorders, intestinal losses, changes in the nutritional metabolism, inflammation severity, nausea, vomiting, abdominal pain-related consumption reduction/augmentation of demand, steroids negative effects on vitamins and minerals, GI’s holding place, the length of the sick bar, resecation and again the disease’s inflammation time/frequency, nutritional insufficiency, protein, vitamin, mineral, element loss. The dietary treatment of CH is a dynamic and dynamic process, depending on the developing complications, the remision and the active period. The purpose of nutrition therapy should be to reduce attacks, maintain the body’s composition during the active period, prevent and replace the loss of macro and microfoods, improve the quality of life. This article discusses the fact that a patient who is diagnosed with CH is implementing different nutritional therapy strategies based on developing complications.
