Diaphragmatic hernias are frequently encountered congenital malformations arising from diaphragmatic pathologies emerging between 3rd and 12th weeks of gestation. They are symptomatic at an early age. Acquired diaphragmatic hernias seen in the elderly are generally associated with trauma. Morgagni hernia is a congenital defect which occurs in the anteromadial region and is rarely seen in the elderly. A 32-year-old male patient who was evaluated with a complaint of “ongoing coughing” was referred to our department upon detection of a mass in the left hemithorax. As a result of radiological examinations, evaluation of initial diagnoses as “low grade liposarcoma, and lipomatosis” were suggested. During the video thoracoscopy, the patient who did not have any history of trauma was diagnosed with Morgagni hernia and the defect was closed with thoracotomy. Since Morgagni hernia is rarely encountered among young people and the differential diagnosis is difficult, our case was deemed suitable for presentation.
