Yetişkin Hastalarda Üst Gastrointestinal Sistemde Yabancı Cisimlerin Yönetimi: Retrospektif Tek Merkez Deneyimi

Giriş ve Amaç: Gastrointestinal sistemde yabancı cisimler dünyada ve ülkemizde ciddi mortalite ve morbidite nedenidir. Genellikle endoskopi ile çoğu vakada tedavi sağlanır. Bu çalışmada amaç üst gastrointestinal sistemde yabancı cisimlerin klinik, epidemiyolojik karakteristiklerinin tanımlanması, yerleşim özelliklerinin belirlenmesi, yapılan endoskopik tedaviler ve sonuçlarının incelenmesidir. Gereç ve Yöntem: Çalışma Ocak 2016-Aralık 2019 tarihleri arasında Gastroenteroloji Bilim Dalında üst gastrointestinal sistemde yabancı cisim nedeniyle endoskopi yapılan hastaları içermektedir. Yabancı cismin özelliği, lokalizasyonu, komplikasyon varlığı ve tedavi şekline hastalara ait özofagogastroduodenoskopi raporlarından ulaşılmıştır. Hastane elektronik veritabanı kullanılarak hastaların yaş, cinsiyet, mental durumu, endoskopinin yapılma zamanı, semptomları, mortalite durumu ile ilgili bilgilere ulaşılmıştır. Bulgular: Çalışmadaki 43 hastanın 16’sı kadın (%37.2), yaş ortalaması 52.2 yıl (22-86) idi. Hastaların 16'sında disfaji (%45.7) en sık semptom olarak saptanmıştır. Yabancı cisimler en sık olarak 29 hastada özofagusta (%67.4) ve 8 hastada (%18.6) midede saptanmıştır. Endoskopi sırasında saptanan yabancı cisimlerin 28’i gıda (%65.1), 6'sı metal (%14) ve 9'u diğer (%20.9) yapıda görülmüştür. En sık tercih edilen endoskopik tedavi hastaların 21’inde fare dişli forseps (%48.8) ile yabancı cisim çıkarılmasıdır. Hastaların 13’ünde komplikasyon gelişmiş olup (%30.2), en sık komplikasyon olarak laserasyon saptanmıştır. Sonuç: Gastrointestinal sistemdeki yabancı cisimler genellikle cerrahi müdahale gerek¬tirmeyip, erken endoskopik müdahale, yutulan yabancı cismin tanı ve tedavisinde en sık kullanılan, güvenli ve etkili yöntemdir.

Management of Foreign Bodies in the Upper Gastrointestinal System in Adult Patients: Retrospective Single Center Experience

Introduction and Purpose: Foreign bodies in the gastrointestinal system are the cause of serious mortality and morbidity in the world and in our country. In most cases, treatment is provided with endoscopy. The aim of this study is to identify the clinical, epidemiological characteristics of foreign bodies in the upper gastrointestinal system, identify the settlement characteristics, study of endoscopic treatments and results. Tools and Methods: The study includes patients who have been endoscoped due to foreign bodies in the upper gastrointestinal system in the Gastroenterology Science Department between January 2016-December 2019. The characteristics, localization, the presence of complications and the way of treatment of a foreign body have been obtained from patients’ ozofagastroduodenoscopy reports. The electronic database of the hospital has been obtained with information about the patient’s age, gender, mental condition, time of endoscopy, symptoms, mortality status. Results: 16 of the 43 patients in the study were women (37.2 percent), the average age was 52.2 years (22-86) In 16 patients, dysphagia (45.7%) was diagnosed as a frequent symptom. As a foreign body, 29 patients were diagnosed with ozofagusta (67.4%) and 8 patients (18.6%) in the stomach. 28 of the foreign bodies detected during endoscopy were found in food (65.1%), 6 in metal (14%) and 9 in other (20.9%). The most common endoscopic treatment in 21 patients is the removal of foreign bodies with mouse dental forseps (48.8%). In 13 patients, the complication developed (30.2%) and laseration was detected as a frequent complication. Outcome: Foreign bodies in the gastrointestinal system are often not required surgery, and early endoscopic intervention is a safe and effective method used in the diagnosis and treatment of ingested foreign bodies.
