Şizofreni tanılı hastalarda çocukluk çağı travmasının dissosiyasyonla ilişkisi

Abstract:

Amaç: Dissosiyatif belirtiler, şizofreni de dahil olmak üzere pek çok psikiyatrik hastalıkla beraber görülür ve sıklıkla öncesinde travmatik bir deneyim mevcuttur. Bu çalışmayla Şizofreni tanılı hastalar’da çocukluk çağı travmasının sıklığı ve dissosiyatif yaşantılarla ilişkisinin araştırılması amaçlanmıştır. Gereç ve Yöntem: DSM-IV tanı kriterlerine göre şizofreni tanısı alan 40 kadın, 40 erkek olmak üzere 80 hasta ile görüşülmüştür. Çalışmaya dahil edilen hastalar sosyodemografik veri formu, SCID-I ölçeği, çocukluk çağı travma ölçeği (CTQ-28), dissosiyatif yaşantılar ölçeği (DES), DES takson ölçeği (DES-Takson), Calgary şizofrenide depresyon ölçeği (CŞDÖ), dissosiyatif bozukluklar görüşme formu (DDIS) ile değerlendirilmişlerdir. Bulgular: Çalışmaya dahil edilen hastalar çocukluk çağı travması olan ve olmayan şeklinde iki gruba ayrıldı. Çocukluk çağı travmasına maruz kalan grup ile maruz kalmayan grup karşılaştırıldığında sosyodemografik veriler açısından anlamlı bir farklılık saptanmadı. Hastalığa ilişkin veriler ile de travma yaşantısı arasında anlamlı bir ilişki gözlenmedi. Travma yaşantısı ile depresyon ve borderline kişilik özellikleri varlığı özellikle ilişkili bulundu, yaşanan travma türlerinden ise duygusal kötüye kullanım varlığı, çocukluk çağı travma ölçeği (CTQ-28) toplam skorundan sonra dissosiyatif yaşantıların varlığı ve depresyon ile en ilişkiliydi. Sonuç: Bu çalışmanın sonucuna göre şizofreni tanılı hastalarda psikiyatrik muayene içinde rutin travma öyküsü sorulması önem arz etmektedir.

Relationship with dissociation of childhood trauma in patients diagnosed with schizophrenia

Purpose: Dissociative symptoms are associated with many psychiatric diseases, including schizophrenia, and often there is a previous traumatic experience. This study aims to investigate the frequency of childhood trauma in patients diagnosed with schizophrenia and the relationship with dissociative experiences. Method and method: 40 women diagnosed with schizophrenia according to the DSM-IV diagnostic criteria and 80 patients, including 40 men, were examined. The patients involved in the study were evaluated by the sociodemographic data form, SCID-I scale, childhood trauma scale (CTQ-28), dissociative life scale (DES), DES taxon scale (DES-Takson), Calgary schizophrenia depression scale (CISD), dissociative disorders conversation form (DDIS). Results: Patients involved in the study were divided into two groups in the form of childhood trauma and non-trauma. Compared to the group that was exposed to childhood trauma with the group that was not exposed, there was no significant difference in social and demographic data. There was no significant link between the disease and the trauma. The trauma life and the presence of depression and borderline personality characteristics were related, while the trauma types experienced were the presence of emotional abuse, the childhood trauma scale (CTQ-28) after the total score of the presence of dissociative experiences and depression. Result: According to this study, it is important for patients with schizophrenia to be asked for a routine trauma history during psychiatric examination.

Purpose: The iam os this study was to search for frequency of childhood trauma in schizophrenics linking the relationship of depression and dissosiciative experience. Materials and Methods: 80 patients (40 male-40 female) were interviewed according to the DSM-IV Patients were evaluated according to the sociodemographic data form, SCID-I form, Childhood Trauma Questionaire (CTQ-28), Dissosiative Experiences Scale (DES), Taxon form of the scale (DES-Taxon), Calgary Depression Scale for Schizophrenia (CDSS) and Dissociative Disorders Interview Scale (DDIS). Results: Patients were divided into 2 groups as who have and who do not have childhood trauma. There was no significant difference between the groups in terms of sociodemographic data. There was also no significant relationship between illness related data of schizophrenia and the trauma experience. İt is specifically observed that trauma experience has relation with depression and borderline personality. The existence of emotional abuse is the most correlated trauma type with dissociative experience and depression after total score in childhood trauma questionaire. Conclusion: It is important to ask for a history of trauma in routine psychiatric examination in patients with schizophrenia.
