Epidermoid cysts are the most common cysts of the skin. They occur in the head and neck with an incidence ranging from 1.6% to 6.9%. These are generally small sized cysts. The cysts rarely reach a size of bigger than 5 cm in diameter. An epidermoid cyst is usually asymptomatic unless it becomes infected or has enlarged enough to produce pressure effects on surrounding structures. There are several theories in order to explain the pathophysiology of the epidermoid cysts. The commonly accepted one is that; they originate from the median pluripotent tissue incarcerated during the fusion of the first and second branchial arch in the third and fourth week intrauterine life. Ultrasonography, computed tomography and magnetic resonance imaging may be used in the preoperative diagnostic work up. Definitive treatment is the total excision of the cyst. In this article, a case of 31 year-old male patient with a 6.7 cm diameter epidermoid cyst which extends from midline of neck to the floor of mouth was presented; and also the clinical symptoms, the differential diagnosis, the imaging techniques useful for diagnosis, and treatment options of dermoid cysts were discussed.
