Amaç Aseptik menenjit ve ensefalit olarak iki genel kategoride incelenen santral sinir sistemi (SSS) viral enfeksiyonları ciddi sonuçları olan ve mutlak erken tanı ve tedavi gerektiren durumlardır. İnsan Herpes virüslerinin bu enfeksiyonların etiyolojisinde önemli rolü olduğu gösterilmiştir. Bu çalışmada, bölgemizde viral SSS enfeksiyonları düşünülen olgulardan gönderilen BOS örneklerinde Herpes Simplex virüsü (HSV-1/2) sıklığı ve incelenen beyin omurilik sıvısı (BOS) örneklerinin biyokimyasal özelliklerini ve aynı zamanda bakteriyolojik kültür üremelerinin de irdelenmesi amaçlanmıştır. Materyal- metod Ocak 2016-Aralık 2017 tarihleri arasında laboratuvarımıza gönderilen 110 adet BOS örneği çalışma kapsamında değerlendirildi. BOS örneklerinden nükleik asit izolasyonu spin kolon yöntemi ile QIAmp® DNA Mini Kit (Qiagen, Hamburg, Almanya) kullanılarak yapıldı. Artus® HSV ½ QS-RGQ kiti ve Rotor-Gene Q 5Plex HRM (Qiagen, Hamburg, Almanya) real-time PZR sistemi kullanılarak BOS örnekleri HSV yönünden kalitatif olarak incelendi. BOS örneklerinde protein ve glukoz ölçüm değerleri de kaydedilmiştir. Ayrıca gönderilen BOS örneklerinin eş zamanlı olarak bakteriyolojik kültürleri de yapıldı. Bulgular Laboratuvarımıza gönderilen örneklerin 5’inde (% 4.5) HSV-1 pozitif saptanırken, HSV-2 tespit edilmedi. BOS’un biyokimyasal paremetreleri değerlendirildiğinde; örneklerin 42’sinde (%38) BOS glukoz düzeyleri, 27’sinde ise (%25) ise BOS protein değerleri normal düzeylerde saptandı. HSV-1 pozitif olan 4 hastanın BOS glukoz düzeyi yüksek bulundu. Protein düzeyi ise 3 hastada yüksek bulundu. HSV ½ negatif olduğu tespit edilen 8 farklı örnekte kültürde üreme saptandı. Sonuç SSS enfeksiyonlarında mortalite ve morbidite oranı yüksek olduğundan etkenin çok hızlı tanımlanması sonrasında spesifik antiviral tedaviye başlanılması hayati öneme sahiptir. Real-time PZR gibi moleküler testlerin kullanıma girmesi viral etkenlerin tespitini kolaylaştırmaktadır. Herpes virüslerinin neden olduğu ensefalit vakaları ile daha sık karşılaşılsa da diğer viral ve bakteriyel etkenler de akılda tutulmalıdır. Bu nedenle aynı örnekte çok sayıda viral ve bakteriyel ajanın aynı anda ve kısa sürede saptanabildiği multipleks PZR paneli gibi moleküler testlerin gerekliliği ön plana çıkmaktadır.
Aseptic meningitis and encephalitis are two general categories of central nervous system (SSS) viral infections that have severe consequences and require absolute early diagnosis and treatment. Human herpes viruses have been shown to play an important role in the etiology of these infections. In this study, the objective of the BOS samples in the cases of herpes simplex virus (HSV-1/2) sent from cases considered viral SSS infections in our region is to evaluate the biochemical characteristics of the samples of the brain spinal fluid (BOS) and also the bacteriological culture reproductions. The material-methode 110 BOS samples sent to our laboratory between January 2016 and December 2017 were evaluated within the framework of the study. Nuclear acid isolation from BOS examples was made using the spin column method of QIAmp DNA Mini Kit (Qiagen, Hamburg, Germany).®Artus HSV 1⁄2 QS-RGQ kit and Rotor-Gene Q 5Plex HRM (Qiagen, Hamburg, Germany) using real-time PZR system, BOS samples were qualitatively examined in the HSV direction.®In BOS samples, protein and glucose measurement values are also recorded.Simultaneously, bacteriological cultures of the BOS samples were also made.In 5 of the samples sent to our Laboratory (4.5%) HSV-1 was found positive, while HSV-2 was not detected.When the biochemical parameters of BOS were evaluated; in 42 of the samples (38 percent) the BOS glucose levels and in 27 (25 percent) the BOS protein values were determined at normal levels. 4 patients with HSV-1 positive were found to have a high BOS glucose level. Protein levels were high in three patients. In 8 different samples found that HSV 1⁄2 was negative, reproduction in culture was detected. As the result is that the mortality and morbidity rates in SSS infections are high, it is vital to start specific antiviral treatment after the very fast detection of the factor. The use of molecular tests such as real-time PZR facilitates the detection of viral factors. Although the cases of encephalitis caused by herpes viruses are more frequently encountered, other viral and bacterial factors should also be taken into account. Therefore, the need for molecular tests, such as multiple PZR panels, in which a large number of viral and bacterial agents can be detected simultaneously and shortly in the same sample, is in the forefront.
