Giriş ve Amaç: Hekime yönelik şiddet olayları giderek artmaktadır ve bunların olumsuz sonuçlarından sadece hekimler değil henüz eğitim aşamasında olan hekim adayları da etkilenmektedir. Klinik ortamda karşılaştıkları şiddet olaylarıyla başa çıkma konusunda eğitilmemiş adayların daha eğitim döneminde bir takım sıkıntılar yaşama ihtimali vardır. Bu çalışmada hedeflenen klinik dönem öğrencilerinde hekime yönelik şiddet, hasta beklentilerini karşılama ve stresle başa çıkma becerileri arasındaki ilişkiyi araştırmaktır. Gereç ve Yöntem: Katılımcılar İstanbul’daki bir Tıp Fakültesi’nde okuyan 5. ve 6. sınıf öğrencileridir. Araştırma anketinde demografik bilgiler, şiddet deneyimleri ve şiddetin nedeniyle ilgili atıfları ortaya koyan sorular, Hasta Beklentilerini Yönetme Becerisi Ölçeği ve Başa Çıkma Stilleri Ölçeği Kısa Formu bulunmaktadır. Analizler SPSS 15.0 ile yapılmış, ortalama, frekans dağılımları hesaplanmış, Spearman korelasyonu, Ki kare testi, Mann-Whitney U Test’i uygulanmıştır. Bulgular: Kişilerin kendisinin şiddete maruz kalıp kalmaması/şahit olup olmaması cinsiyete göre değişiklik göstermemiştir. Öğrencilerin şahit oldukları şiddet olayına müdahale etmeme oranı %78.7’dir. Katılımcıların %56.8’i ileride şiddete maruz kalma ihtimaliyle ilgili endişe seviyesini epey/son derece olarak bildirmiştir. Yüz yetmiş dört kişi (%82.9) şiddet olaylarının hekimlik eğitimini olumsuz yönde etkilediğini bildirmektedir. Öğrencilerin günlük hayatta sağlık ve hekimlikle ilgili konuşmalarının yaklaşık %16.50’si şiddetle ilişkilidir. Kadınların erkeklere göre, ileride şiddete maruz kalmayla ilgili endişeleri daha yüksektir. Şiddetin nedeniyle ilgili atıflar genelde dışsal niteliktedir. Madde kullanma şeklindeki başa çıkma stratejisi erkeklerde daha yüksek orandadır. Sonuç: Tıp fakültesi öğrencileri, daha eğitim döneminde şiddete maruz kalmakta, şiddet olaylarına müdahale edememekte ve dolayısıyla bu olayların yarattığı endişeyi hem öğrenme süreçlerinde hem de geleceğe projekte ederek deneyimlemektedir. Şiddeti sezmek ve yatıştırmak, daha lisans eğitimi döneminde kazanılması gereken klinik becerilerdendir.
Introduction and Purpose: The incidents of violence against the doctor are increasing and their negative consequences are affected not only by the doctors but also by the doctors candidates who are still in the training stage. The candidates who are not trained to cope with the violence they encounter in the clinical environment are likely to experience a number of trouble during the more educational period. The aim of this study in the clinical period is to explore the relationship between the violence towards the doctor, the patient’s expectations and the ability to cope with stress. Tools and Methods: Participants are 5th and 6th grade students studying at a Medical Faculty in Istanbul. The survey includes demographic information, violence experiences and questions that give references to violence, patient expectations management skills scale and short form scale of approach styles. Analysis was done with SPSS 15.0, average, frequency distribution calculated, Spearman correlation, Ki square test, Mann-Whitney U test applied. The findings: whether or not the persons themselves were exposed to violence or not witnessed has not changed according to gender. Backgrounds: Violence towards the doctors has been increasing and the doctors/medical students experience the negative effects of them. Candidates not trained to cope with these events have a probability to experience distress in the undergraduate education. The aim is to investigate the relationship among violent experiences, satisfying patient expectations and coping styles in clinical years. Material and Method: The participants are the 5th-6th year medical students in Istanbul. The research questionnaire includes questions for demographics, violent experiences, attributions for violent events, Managing Patient Expectation Scale and Cope Inventory. SPSS 15.0 was used to calculate means and frequencies, Spearman correlation were calculated and Chi-square test and Mann-Whitney U Test were used for comparisons. Results: Either being a victim of a violent event or witnessing it were not changed as a function of gender. The students did not intervene in these events (78.7%). 56.8% of the participants’ anxiety level was high/very high for being a victim of a violent event as a doctor in the future. 174 people indicated violent events had negative effects on their education. Violence related topics compromised 16.50% in students’ daily conversations. Women were more anxious than men for future victimization possibility. Casual attributions of violence were mostly external. Substance use as a coping strategy was more widespread in men than women. Conclusions: Medical students experience violent events, cannot interfere with them and project their anxiety to the future professional life. Sensing and alleviating the violent interactions are among the clinical skills that must be learned in the undergraduate education.
